ANNEX 2 FY 2014 PUBLIC HEALTH MASTER AGREEMENT ANNEX Program Description and Reporting Requirements

PROGRAM NAME: Babies Can’t Wait (BCW) Georgia’s Individuals with Disabilities Education Act Part C Early Intervention System

PROGRAM CODE: 112/543

FUNDING SOURCE: 543/Office of Special Education Programs (OSEP) Part C of the Individuals with Disabilities Education Act (IDEA) Infant Toddler Grant 112/State Budget

PURPOSE: To provide early intervention services and supports to infants and toddlers with special needs and their families.

FUNDING REQUIREMENTS:

Restrictions:

The BCW funds distributed to each contractor are to be used to support the early intervention system for BCW services. Collaboration and coordination with the Children 1st program is required.  Infant Toddler Grant and state funds must only be used to provide services and supports for and related to infants and toddlers birth to age three

Fiscal:  Funds from program codes 543 and 112 can be used interchangeably to support this annex. The transfer of funds between Public Health program codes is not allowable.  Indirect costs shall not be charged unless a cost allocation plan is approved by the Office of Financial Services  Districts with mid-year budget expenditures <40 percent of the total BCW budget allocation, may be adjusted and reallocated by the State Office.  Funds not utilized by the end of the fiscal year must be returned to the state office  All outsourced printing and copying expenditures must be supported by three (3) separate quotes in order to ensure funds are utilized to purchase only the most economical services.  Contractors must have prior approval from the state office for individual equipment purchases and individual lending library purchases over $1,000 and all “unspecified costs”. Requests must be submitted using the (Request to Purchase Equipment Form). Failure to obtain prior approval may result in charges being disallowed. No individual purchase over $1,000.00 can be made without state office approvals. *All equipment over $1,000 each must be in compliance with the master agreement.  Contractor shall obtain state office approval before any BCW funds are spent on rent.  Fees generated by the program, including Diagnostic, Screening, and Preventive Services (DSPS) billing, must be returned to, and used to support the activities of the program generating the fees. Funds shall not be diverted into other activities or programs.

Deliverables:  Adhere to the IDEA Federal Regulations and requirements/Georgia Part C Babies Can’t Wait Program Policies and Procedures  Implement the Primary Service Provider (PSP) teaming model for all families enrolled in the local Babies Can’t Wait (BCW) program.  Participate in any site reviews or desk audit monitoring activities (dates TBD)  The contractor shall comply with specific reporting requirements for each program and submit reports to the state office. The contractor shall ensure that 100% of data reported to the state office programs are accurate and submitted by the required deadlines. County level data shall be provided if requested by the State Office.  The BCW District Coordinator shall participate in all scheduled coordinators’ meetings with state office staff throughout the year. If unable to attend, an alternate must participate in the meeting  The BCW District Coordinator must assure the implementation of the early intervention system and coordination among other child health public health programs.  Use the state-approved data system to track, monitor and follow-up on all infants and children referred to and served by the program. All services and claims must be entered into the Babies Information and Billing System by all service coordinators and providers contracted to provide services to the eligible children and families.  Provide training and technical assistance, as identified or requested, to district staff, contractors and community referral sources regarding the program.  Respond to any finding requiring a corrective action within 60 days from the date the corrective action report is received. All findings of noncompliance must be corrected within 12 months from the date of notification.

REPORTING REQUIREMENTS:

 The data transmissions for Babies Can’t Wait LICC and Public Awareness Reports are due quarterly. For due dates, please refer to the Quarterly Report Schedule in the GIA annex for program code 112 and 543. Reports are to be submitted to the State Office electronically via email to Ruth Cantor@[email protected]. See Attachment A
