The post will be based at the Royal Hospital for Sick Children, Glasgow, a national leader in the secondary and tertiary health care of children and families. Plans are underway for a move to a new children’s hospital, located on the Southern General Hospitals campus and due to open in 2015.

This is a new post joining a team of three established Consultants (two NHS Consultants and one Senior Lecturer) delivering paediatric rheumatology care for children in the West of Scotland and supporting services around the whole of Scotland. The department has an established multidisciplinary team with dedicated nurse specialist, physiotherapy and occupational therapy time.

The paediatric rheumatology service in Glasgow is at the centre of a Managed Clinical Network (Scottish Network for Paediatric and Adolescent Rheumatology, SPARN) for paediatric rheumatology services in Scotland. It is actively involved in both undergraduate and postgraduate teaching. It is an active participant in many UK-wide multi centre research studies and has a locally based research programme led by the Clinical Academic.

The post will be based in Glasgow, providing leadership for the clinical service in Glasgow. Opportunity exists for the appointed individual to develop areas of special interest within the service and interests complementing those of the current Consultants would be welcomed.

Application is therefore sought from an individual with specialist training in Paediatric rheumatology (National Grid or equivalent) .

Applicants must have full registration with the General Medical Council, a licence to practice and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher specialist training leading to CCT or eligibility for specialist registration (CESR) or be within 6 months of confirmed entry from date of Interview. Applications are invited for a whole time (or maximum part time ) post of Consultant in Paediatric Rheumatology. This is a new post created principally to support the Paediatric Rheumatology service based at the Royal Hospital for Sick Children, Glasgow. There is also a commitment to support other services working within the Scottish Paediatric and Adolescent Rheumatology Network (SPARN). The appointee will work with the current team of Consultants (2 Consultants and 1 Senior Lecturer in Paediatric Rheumatology) working within the developing Scottish Paediatric and Adolescent Rheumatology Network. This team supports Paediatric Rheumatology Services within Scotland with major services in Glasgow and Edinburgh and outreach clinics in many areas of Scotland. Opportunity for the development of an individual candidate’s special interest will be encouraged within the service.

Children’s Services are managed within the Women & Children’s Directorate, currently located within the Yorkhill Campus which hosts the Royal Hospital for Sick Children. Neonatal services within the city are provided on two sites; the Southern General Hospital and Princess Royal Maternity Hospital. Child and Adolescent Psychiatry facilities include an inpatient child psychiatry unit.

The Royal Hospital for Sick Children, Glasgow

The Royal Hospital for Sick Children, Glasgow is the largest paediatric teaching hospital in Scotland. It provides care, not only for children resident within Greater Glasgow & Clyde, but is also a tertiary referral centre for children from the West of Scotland, and in some subspecialties, from the whole of Scotland.

There are 260 inpatient and day care / assessment beds within the hospital. There is a dedicated medical day case unit.

There is an integrated 26-bedded intensive / high dependency critical care unit and a retrieval service for critically ill children. Adjacent to the critical care services in the hospital is a large theatre suite.

All paediatric medical and surgical subspecialties are represented, including general medical paediatrics, respiratory, nephrology, endocrinology, gastroenterology and nutrition, haemato-oncology, cardiology, neonatology, immunology and infectious disease, dermatology, rheumatology, metabolic medicine, audiology, ophthalmology, ENT surgery, orthopaedic, plastic surgery, maxillo-facial and dental surgery and general paediatric and neonatal surgery. The hospital provides a national Extra Corporeal Life Support service and is the centre for all paediatric cardiac surgery in Scotland.

The Radiology Department located within RHSC provides ultrasound, CT, MRI and isotopic studies on site. PET scanning is available within Glasgow. Diagnostic laboratory facilities are on site in Haematology, Blood Banking, Biochemistry, Microbiology, Virology, Histopathology and Genetics.

There is an extensive range of specialist community based children’s services across NHS Greater Glasgow. Managed within community health and social care partnerships, these services are integrated with primary care and social care services. Well established clinical links across combined acute and community settings within the NHS Board are in place.

Neonatal facilities in GG&C are based at the Princess Royal Maternity hospital, Southern General Hospital, RHSC and RAH Paisley. Paediatric neurosurgery is based in the Institute of Neurosciences at the Southern General Hospital but will transfer to RHSC for early 2012.

The Hospital provides the major Undergraduate Paediatric Teaching facility for the University of Glasgow and accommodates the University Departments of Child Health, Child and Family Psychiatry, Medical Genetics, Human Nutrition, Paediatric Pathology, Paediatric Biochemistry and Paediatric Surgery. The Yorkhill academic campus, including Child Health, is part of the School of Medicine. A Senior Lecturer in Paediatric Rheumatology was appointed to the department in 2002. The hospital is a major undergraduate and postgraduate teaching centre for paediatrics.

New Hospital Build

In 2004 there was ministerial announcement for the development of a new children’s hospital (replacing the current Royal Hospital for Sick Children) to be built within the Greater Glasgow and Clyde area, this build to be co-located within acute adult facilities. The new children’s hospital will be located on the Southern General Hospitals campus, together with a new adult hospital and a refurbished, extended maternity hospital.

Planning for this new build is at an advanced stage and the new Children’s Hospital will open in 2015.

2. Paediatric Rheumatology Service in Glasgow

The paediatric rheumatology service in Glasgow sees around 300 new patient referrals per annum. Referrals come mainly from Glasgow and the West of Scotland but at times from as far afield as the Western Isles. The service is at the centre of the now established Scottish Paediatric and Adolescent Rheumatology Network, with children from around Scotland accessing the Glasgow service for part of their care while the majority is delivered by local teams.

There are two dedicated paediatric rheumatology clinics per week, one for primary school age children and one for teenagers. There is a monthly connective tissue diseases clinic and a monthly multidisciplinary hypermobility clinic. Children with uveitis are managed in a combined service with the ophthalmologists.

There is an excellent dedicated multidisciplinary team comprising 1.2 nurse specialists, dedicated physiotherapy and occupational therapy. There is a dedicated physiotherapy department with its own hydrotherapy pool. There is access on site to podiatry, orthotics, psychology and liaison psychiatry and a dedicated children’s pain team.

There is an excellent medical day unit providing care for children requiring day case admissions for infusions etc. There is a fortnightly general anaesthetic joint injection list with facilities for entonox joint injections in both the day unit and the outpatient department. The radiology department provides on site access to all imaging including MR, CT, DEXA and isotope scanning. PET scanning is available in the city. We provide our own musculoskeletal ultrasound within the rheumatology service for diagnostic purposes, and guided placement of corticosteroid. The paediatric rheumatology department has a dedicated state of the art ultrasound machine.

There is a weekly multidisciplinary team meeting, monthly team education sessions which support education across the network, and a monthly radiology meeting.

The team at RHSC includes the Senior Lecturer in Paediatric Rheumatology, a post funded by Arthritis Research UK which has established a successful research programme, alongside the clinical paediatric rheumatology service for children in Scotland. A number of research projects have developed around a central theme “Identifying risk factors for poor long term outcome of childhood onset chronic rheumatic disease” with interest in cardiovascular risk (collaborations within the University of Glasgow with Prof N Sattar, Prof I McInnes, Prof J McColl), musculoskeletal ultrasound and imaging (collaborations with Birmingham (Dr K Johnson) and Glasgow Caledonian University (Dr D Turner) and foot care (Prof J Woodburn, Glasgow Caledonian University) bringing funding of over £1 million from the Chief Scientists Office of the Scottish Executive, two Barbara Ansell Fellowships and a number of project grants from ARUK since 2002 and bringing MSc and PhD students to the department.

The whole rheumatology team contributes to a number of current UK-wide multicentre projects in paediatric rheumatology including the Childhood Arthritis Prospective Study, the UK JSLE Cohort Study, the BSPAR Biologics Registries, Juvenile Dermatomyositis Cohort Study, SYCAMORE (adalimumab for JIA uveitis) and POPS (paediatric osteoporosis) study.

Clinical research is supported by the Paediatric Clinical Research Facility housed within Yorkhill, with a similar facility existing in Edinburgh. These facilities support the majority of our clinical research projects, including providing dedicated Research Nurse time.

3. Scottish Paediatric and Adolescent Rheumatology Network (SPARN)

Paediatric Rheumatology services throughout Scotland now work together within a formal Managed Clinical Network. Around 800 children with JIA are now managed within the network.

Locally based multidisciplinary teams and paediatricians with an interest in the specialty manage the majority of children with rheumatological conditions, supported by expert teams in Glasgow and Edinburgh. There is now an established network of shared care clinics, with clinics established currently in Aberdeen, Dundee, Inverness, Lanarkshire, Ayrshire, Clyde, Fife and Dumfries. It is hoped to establish clinics in Forth Valley, Borders and West Lothian in the near future.

The network holds an annual multidisciplinary network meeting and supports a variety of training and educational opportunities for staff. 4. The Job Itself

Consultant Paediatric Rheumatologist. The successful applicant for this new post will join three Consultant colleagues to provide expert Paediatric Rheumatology services in Glasgow and to aid in the further development of a network of clinics providing care throughout Scotland.

The appointee to this new post will i) work as part of a four Consultant team leading paediatric rheumatology services within Scotland ii) provide continuity and clinical leadership for the service in Glasgow iii) support and work with the multidisciplinary team in Glasgow to develop their roles and the clinical service iv) lead the joint injection service within RHSC, Glasgow v) continue the development of network services including support for RHSC, Edinburgh and outreach clinics vi) have a role in both undergraduate and postgraduate teaching and training vii) contribute to multidisciplinary training through the network viii) undertake audit within the service and network ix) support and encourage clinical research

There is a limited general paediatric commitment in this post. The appointee will be expected to provide one general paediatric clinic per week.

The appointment of an additional full time Consultant based in Glasgow to the team will provide more continuity for the clinical service in Glasgow. This will facilitate improved care for complex inpatients by providing a more consistent presence.

It is anticipated that the consolidation of the clinical team with this additional post will facilitate the teams in Glasgow and Edinburgh continuing to work closely together to develop services and lead the network.

The appointment holder will have the opportunity to develop a role supporting a number of network centres within SPARN.

In addition, the appointment to this additional consultant post will facilitate further development of specialist interests within the service.

With current RCPCH recommendations that specialist advice should be available at all times it is possible that telephone availability out of hours may be considered as part of this post in the future. Provisional Weekly Timetable for 10 PA job plan

Programmed Activities in Provisional Job Plan

For a whole-time contract: Direct Clinical Care 9 PAs . Supporting Professional Activities 1 PAs Work Programme/Job Plan

A formal job plan will be agreed between the appointee and their Clinical Director, on behalf of the Medical Director, prior to commencement. The job plan will be based on the provisional timetable shown above. This is for an average of ten PAs.

The Job Plan will then be reviewed annually, following the Appraisal Meeting. The Job Plan will be a prospective agreement that sets out a consultant’s duties, responsibilities and objectives for the coming year. It should cover all aspects of a consultant’s professional practice including clinical work, teaching, research, education and managerial responsibilities. It should include personal objectives, including details of their link to wider service objectives, and details of the support required by the consultant to fulfil the job plan and the objectives. Achieving national waiting time guarantees will be a prerequisite.

Continuous Professional Development

Continuous professional development is supported according to the guidance of the Royal College of Paediatrics and Child Health. The hospital has regular CPD sessions and there is a monthly SPARN CPD afternoon in rheumatology.

Postgraduate teaching and training

The appointee will contribute to postgraduate education in rheumatology in Glasgow throughout the Scottish network. There will also be a variable requirement to teach postgraduates and other professionals including Nurses, Health Visitors and Allied Health Professionals.

Undergraduate Teaching. The appointee will be expected to take part in the teaching and examination of medical students from Glasgow University, and will have an opportunity to contribute to the ongoing development of both paediatric and musculoskeletal components of the undergraduate course.

Audit: The appointee will be expected to contribute to regular audit within the department and network.

Research: Research is encouraged. It is expected that the appointee will be involved in facilitating Glasgow’s involvement in the various UK-wide multicentre research projects that are ongoing. The Senior Lecturer in Paediatric Rheumatology is based in Glasgow and collaborative research working will be encouraged. Section 5 Main Conditions of Service

General Provisions

You will be expected to work with local managers and professional colleagues in the efficient running of services and will share with Consultant colleagues in the medical contribution to management. Subject to the provision of the Terms and Conditions, you are expected to observe the organisation’s agreed policies and procedures, drawn up in consultation with the profession on clinical matters, and to follow the standing orders and financial instruction of Greater Glasgow & Clyde NHS Board, in particular, where you manage employees of the organisation, you will be expected to follow the local and national employment and personnel policies and procedures. You will be expected to make sure that there are adequate arrangements for hospital staff involved in the care of your patients to be able to contact you when necessary.

All medical and dental staff employed by NHS Greater Glasgow & Clyde are expected to comply with the central Health and Safety Policy.

You will have responsibility for the training and supervision of (junior) medical staff who work for you and you will devote time to this activity on a regular basis. If appropriate, you will be named in the contracts of doctors in training grades, as the person responsible for overseeing their training and as the initial source of advice to such doctors on their careers. TERMS AND CONDITIONS OF SERVICE

The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.

GRADE AND SALARY Consultant £ 74,504 £ 100,446 per annum (pro rata)

New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.

HOURS OF DUTY Full Time 40.00

SUPERANNUATION You have the option to join the NHS Superannuation Scheme, to participate in the State Earnings Related Pension Scheme or to take out a Personal Pension. Employee’s contributions to the NHS Scheme are Tiered based on your earnings and the employers contribution equates to 13.5 % of salary. Employees in the NHS Scheme are “Contracted-out” of the State Earnings Related Pension Scheme and pay a lower rate of National Insurance contributions. Employees who choose to participate in the State Earnings Related Pension Scheme pay the higher rate of National Insurance contribution. A Stakeholder Pension is also available. A Personal Pension is a private arrangement agreed with the pension provider that will be an organisation such as a Bank, Building Society or Insurance Company.

REMOVAL EXPENSES Assistance with removal and associated expenses may be given and would be discussed and agreed prior to appointment.
