ESSENTIAL DUTIES, RESPONSIBILITIES AND DESIRED CHARACTERISTICS: A. Stewardship  By following client’s care plan, provides personal care; basic homemaking services; or respite care for client.  Homemaker chores to include but not limited to: meal preparation, cleaning and maintaining home to provide safe environment, laundry and errands.  Personal care to include but not be limited to: bathing, dressing, assisting with toileting and medication reminders.  Advanced personal care duties to include but not limited to: checking vitals, assisting with mobility/transfer, skin and nail care  Provides client services on scheduled day and at scheduled time; if there is a change in plans, informs Senior Care Services office of situation.  Provides client care in accordance with Mid America Regional Council (MARC), Clay County Senior Services, Platte County Senior Services and Department of Senior Services standards and expectations.  Adhere to organizational policies, procedures and practices; as well as laws and regulations.  Achieve expected results and goals. Can be relied upon to fulfill the job assignments in a timely fashion.  Produces high quality work with a focus on accuracy, attention to detail and neatness-this includes following client care plans and accurately completing time sheets.  Uses time effectively to complete work within established time frames.  Demonstrates ability to multi-task and work effectively in a changing environment.

B. Teamwork and Collaboration  Participates in staff meetings and in-service training sessions.  Timely, completion of time sheet/care plan notes as required.  Adheres to safety standards when providing client care.  Facilitates a professional relationship with clients, client’s family members and co-workers.  Communicate to the office any outstanding concerns regarding clients change in health condition(s), family situations, or other circumstances that need to be relayed and documented in file.  Required to communicate with supervisor weekly to discuss client care and care plans.  Exhibits positive behavior when interacting with superiors, co-workers and the general public.  Addresses opportunities and concerns with the appropriate person and in a professional manner.

C. Commitment to Customers  Maintains client confidentiality at all times and conceals information when in the community.  Contacts Senior Care Services office if unable to provide service to client.  Notifies Senior Care Services office if there are changes in client’s condition or home situation.  Is able to run errands/shopping and do laundry off site per clients care plan.  Must be able to demonstrate understanding of written and verbal communication when documenting client services.  Demonstrates knowledge of client/caregiver agreement and adheres to the policies of Catholic Charities.

MINIMUM EDUCATION and/or EXPERIENCE: Must be at least 18 years old with a high school diploma or GED. The ability to read, write and follow care plan directions are required. Reliable transportation, valid driver’s license and current automobile insurance are requirements of the position as well as a clean driving record. Two years of previous experience caring for children or senior citizens is required. Must be skillful in utilizing Microsoft Office.

MINIMUM CERTIFICATES, PROFESSIONAL LICENSES, REGISTRATIONS: Certified Nursing Assistant license for the State of Missouri. Valid driver’s license

MINIMUM TRAVEL REQUIREMENTS: Throughout the greater Kansas City, Missouri metropolitan area, Clay and Platte Counties.
