______I acknowledge that I did not turn my work in on time. I know that my parent(s) / guardian(s) will be contacted this day (day 0) to keep them informed. I also understand that I have been granted an opportunity to replace the zero I have earned with a grade no higher than a 69 if I fulfill the following requirements:

1. The missing assignment must be turned in the very NEXT school day before the end of school, EVEN IF THIS CLASS DOES NOT MEET THAT DAY. 2. A statement of intent for Positive Behavioral Change (see back side) must be signed by the student and parent. 3. This ZAP FORM must be stapled to the late assignment and returned with the late assignment the very next day before the end of school .

If the next day (day 1) passes with no return of the missing work, the highest possible grade earned is reduced to a 60.

If the second class day passes (day 2), with no return of the missing work, the highest possible grade earned is reduced to a 55 and the student’s name is sent to his support counselor at the end of the day.

If the third day passes (day 3), with no return of the missing work, the highest possible grade earned is reduced to a 50

On the fourth day, with no return of the missing work (day 4), the student’s name is sent to his Assistant Principal at the end of the day, A copy of the ZAP log will also be sent. The ZAP (Zeroes Are Prohibited) Form

My Positive Behavioral Change Statement

I recognize that I did not live up to my teacher’s high expectations of my academic progress when I did not turn in my assignment for class. I recognize that one of the many real world values that my teacher is trying to instill in me is to be responsible for producing and turning in my work. I also recognize that my reason for not turning in my work may become an obstacle to my future academic success if I do not act now to change my ways. I will talk with my parent(s)/guardian(s) and teacher about ways to be more prepared for educational success. I plan to take the following steps to improve my work habits:

As a parent/guardian, I recognize the need to help my child maintain good academic habits. One of these habits is to be aware when assignments are due and to turn them in when the teacher indicates the assignments are to be turned in. I will help my child make a positive behavioral change in the following way:
