CNM SONET/MON Form Instructions
Section 1 Provide Name, SBCUID and Phone for primary and secondary Account Team contacts. This information is used for setup notification. Section 2 Yes/No – Check yes if this customer has existing SONET or MON services already monitored by SONET/MON CNM. Company Name – Provide the complete name of the business customer. ACNA/Customer Code – Provide an ACNA if available (uppercase letters only). If none, leave this field blank. Name, Phone, Email & Address – Provide the primary business contact for the CNM service who can be contacted by the CNM Help Desk if needed. Section 3 This section is for AT use only and is NOT used by the Optical Helpdesk for the initial setup of CNM. Section 4 Action Being Requested – Select “Add” for circuits that are currently NOT provisioned on CNM. Select “Enhance” if GUI or Data Feed is being added to an existing circuit being monitored by CNM. Select “Remove” to cancel the service for both GUI and Data Feed Type of CNM Application – Select the appropriate CNM option from the drop down menu. CNM is to be associated with the following products - Select the appropriate service. CNM is available only on the products listed.
Section 5 Circuit Details – Provide the appropriate SCID or DWID (uppercase only) for the customer’s ring. Section 6 Note: List all target IDs (TIDs) in uppercase only. Failure to do so will result in a failed setup in CNM. Gateway NE IP Address and TIDS: o For Fujitsu SONET: Go to http://mwnoc.sbc.com/chooser/ and enter the SCID. A listing of the TIDs for that SCID and the associated NETSMART EMS will be provided. Cut and paste the precise TIDS into the individual fields on this form. Note the server in the left column on the web page and reference the following spreadsheet to find the IP and port for that server:

o For Nortel SONET and MON: Contact your regional Network SME listed at the following link: http://netcool.sbc.com/admininfo/Regional_Network_Contacts.html. They will provide the gateway NE IP address, gateway port, and TIDs for your customer’s ring. Use the following spreadsheet to request the Nortel node information from the Network contact:

The Help Desk will escalate your issue within Network. Section 7 – For Data Feed Customers Only Private Line Order, Order Due Date, VPMO Order # and B2B VPN Due Date are for AT use only. SNMP Receiving Host info – Provide the IP and port number for the customer’s server that will receive SNMP traps. Customer FTP Host info - Provide the IP, login and password for the customer’s server that will receive the CNM FTP files. Frequency of Data Transmission – Recommended interval is 15 minutes. For customer rings with more than 12 nodes, please contact SONET/MON Product Management for interval recommendations. Section 8 – For GUI Customers Only Check Add for new CNM customers or if additional login is requested for an existing customer. Check Remove if the CNM service is being cancelled or an additional login is no longer requested. Provide primary customer contact information for each Web GUI login requested. One login is provided at no additional charge with the CNM service.
