CULTURE SURVEY

Instructions

The following fifteen question survey has been designed specifically for COMPANY X as part of the introduction of our Management Improvement Program. The objective of the survey is to ascertain your perceptions of a variety of issues affecting organisation performance including perceptions of management, job satisfaction, communication, TQM and other general issues. We ask that you be honest in your answers as your response will help the organisation identify critical blockages to success. We do not require identification as your answers will be treated anonymously and in confidence. The results will be tabulated to provide an organisational picture of these issues. Question 15 invites any other comments you would like to make about the organisation that are not specifically covered in the survey.

Do not mark the survey questionnaire, but place your answers (circle or tick as appropriate) on the separate answer sheet. (1) Indicate how you see your supervisor or manager for each of the following statements:
