HEARINGS - Mary Ellen Gillon:

1. REYES, LUIS - LGB.13760 Commission grants Attorney Buzaid’s request for a continuance.

2. ESTEPHAN, ZAHI - LGB.13138 Commission accepts offer in compromise in lieu of hearing of 2 days and $1,050 for incident of 8/24/03.

3. DETELJ, ANJEANE - LCA.6196 Commission accepts offer in compromise in lieu of hearing of $500 for incident of 11/8/03.

4. LOMBARDO, MICHAEL - LCA.6095 Due to imminent sale, commission accepts offer in compromise in lieu of hearing of $1,000 for incident of 9/27/03.

7. LILLANEY, GOPALDAS - LIP.13450 Commission rejects offer of $3,750 for incident of 9/2/03.

8. COX II, ROBERT - LCA.6291 Commission rejects offer of 1 day and $550 for incident of 11/18/03.

9. PATEL, VARSHA - LIP.13480 Commission would accept 5 days and $1,500 for incidents of 9/26/03 and 4/6/04 (second offense of section 30-86).

DISCUSSION - Lynn Fiore:

1. SCASINO JR., NICHOLAS - LIR.14193 Commission requests compliance meeting regarding 1/30/04 investigation. Takes no further action on 30-6-A24(a), smoking in barroom, and would accept 15 day suspension and $4,500 offer in compromise to resolve remaining charges including 5 counts of sale to minors.

2. NANDKUMAR, BADLANI - LIP.13708 Commission takes no further action on one alleged count of sale to minor (Martinez), but will pursue remaining count of sale to minor (Estapa) and definition of package store violation from 3/13/04 investigation. Would accept 3 day suspension and $1,125 offer in compromise to resolve.

3. ST. GERMAIN, MICHELLE - LGB.12392 Commission would accept 3 day suspension and $1,500 fine to resolve alleged violations from 11/3/03 investigation regarding termination of permittee and fraudulent renewals.

4. PIROZZOLI JR., MARIO - LCA.5681 Commission would accept 7 day suspension, $5,000 fine and submission of current sketch of interior to resolve alleged violations from 11/14/03 investigation, including dancer conduct and unapproved dancer locations. 2. DIMARTINO, LESLIE - LGB.13780 Riverton General Store, Barkhamsted Ok to issue provisional permit when in receipt of stipulated agreement, $500 fee, completion of application (Sections M & N signed and notarized by backer).

3. STARK, BRENDA - LCA.6504 Once Again Café, Meriden Ok to issue provisional permit when in receipt of stipulated agreement, $500 fee, bill of sale, sellers affidavit, cancel permit LCA.5884.

4. DAVE, SMITA - LIP.13929 Dave’s Discount Liquors, Norwich Ok to issue provisional permit when in receipt of stipulated agreement, $500 fee, bill of sale, sellers affidavit, cancel permit LIP.12208.

5. ESPOSITO, RONALD - LGB.13816 The Stop & Shop Supermarket, Wallingford Ok to issue provisional permit when in receipt of stipulated agreement, zoning approval.

6. SAVALIA, DILIP - LGB.13808 Dada Deli, Waterbury No action taken by commission. No photos submitted and 9-item breakdown consisted of estimates.

7. YASMER, SAMUEL - LIR.16715 Geer Village, North Canaan Ok to issue provisional permit when in receipt of stipulated agreement, sellers affidavit, copy of lease/deed, cancel permits LCO.1457 & LCO.1360. Liquor Control Minutes August 10, 2004 Page 4 of 7

1. RODGERS, BRIAN - LCA.6459 Dew Drop Inn, Derby Commission approved request for an extension to provisional permit.

2. REVIS, NIKOS - LIR.16615 West Main Pizza, Plainville Commission approved request for an extension to provisional permit.

FINAL PERMITS:

1. BARNETT, HANNELORE - LSL.922 Valckenberg International, Tulsa, OK No requirements. Ok to issue final permit.

2. CALDARELLA, LEO - LIB.45 Princess Pintail, Old Saybrook No requirements. Ok to issue final permit.

3. CLARK, WESLEY - LIR.16704 Voodoo Grill, Mystic Ok to issue final permit when in receipt of proof of lockable storage. Endorse patio.

4. MASLIAH, DAVID - LIR.16666 Zinc Bistro & Bar, Stamford Ok to issue final permit when in receipt of completion of application (Section B), health certificate, photos of exterior and furnished interior.

5. ADALFO, ROSA - LIR.16630 Caribbean Bar & Restaurant, Willimantic Assigned to a hearing for section 30-22(e), definition of a restaurant, and section 30-6-B29, proper separation. If approved, still need bill of sale, FEIN, sales tax permit, sketch with dimensions, proof of proper separation and proof of food.

CHEN, SHU FA - LRW.4151 Asahi Japanese Restaurant, Rocky Hill Ok to issue final permit when in receipt of substitute permittee application filed and approved by commission (applicant unable to read and understand English), information release for Li Ming Ke signed and notarized, employee list, equipment list, proof of lockable storage. 2. GUO, CHENG - LIP.13837 South Main Street Wines & Liquors, Middletown Commission discussed request in letter dated June 16, 2004 from Brian Robinson, zoning officer, City of Middletown. Zoning officer signed application and permit was issued on June 2, 2004. Commission, therefore, refused request in letter dated June 16, 2004.

3. LONG, LETA - LCA.6163 The Brookside, Preston Commission approved all proposed ACB locations.

4. REGENSBURGER, JOSEPH - LCA.6201 Jack’s, Bridgeport Commission approved new lockable storage room.

5. SILVA, JACK - LIR.16267 Carmelina’s Restaurant/Lounge, Bridgeport Commission approved request for extension of premises. Need fire department approval.

6. ALMONTE, LUIS - LGB.12906 Tropical Supermarket, Hartford Commission approved transfer of stock and substitute permittee. Need copy of promissory note.
