I agree to the following for the next _____ years.

1. Remove trash and debris at the weir crest and auxiliary spillway.

2. Repair any scouring at the weir crest.

3. Repair any holes along structure caused by rodents to prevent potential undermining of the structure.

4. Remove any trees, brush, or other obstructions at the outlet channel and apron.

5. Repair any cracked, broken, or defective concrete or metal.

6. Repair any excessive scour at the apron in the downstream channel.

7. Do not install fences or other devices across the weir crest and auxiliary spillway that may restrict storm flow.

8. Maintain adequate vegetation on the berms, outlet channel, and borrow area as recommended in Wisconsin Job Sheets 134, How to Establish and Maintain Introduced Grasses and Legumes, and 135, How to Establish and Maintain Native Grasses, Forbs and Legumes.

9. Exclude livestock from berms, auxiliary spillway, and concrete apron.

10. Follow the planned Resource Management Systems to reduce sediment accumulation.
