2. Minute of Meeting held on 13th September 2013

The minutes of the meeting held on 13th September 2013 were accepted as a true record.

The Chair welcomed everyone to the meeting and welcomed Graham Taylor to his first meeting, representing the Scottish Episcopal church. Introductions were made around the table.

3. Matters Arising

3.1 NHS Grampian Spiritual Care Policy

The NHS Grampian Spiritual Care Policy was approved at the last meeting in September 2013. Any outstanding issues are currently being dealt with before the policy is submitted to the Clinical Review Policy Group; thereafter the policy can be added to the document silo.

3.2 Sarah’s story

Jim reported that Sarah has not been admitted to hospital since her story was reported to this group. Pamela Adam has spoken with Sarah and her family and they are happy with outcome and are pleased Sarah’s story and experience may help other people. Pamela assured the family that learning has resulted from their experience.. Guardianship is now in place for Sarah and her younger brother. Sarah was presented will a small gift of appreciation.

3.3 ARI Mosque

Laura confirmed work is still ongoing to improve facilities at the Prayer room (ARI Mosque). The Mosque Working Group met and came up with options to improve facilities:

1. To erect a semi-permanent awning along the elevation of the building at a cost of £6,000.

2. To build a two bay extension at a cost of £102,000 or three bay extension at the cost of £123,000. .

Discussion took place around the effect this building work would have on the usage of the Prayer room and that other accommodation would require to be sought during the construction period.. Mark suggested that the ARI Chapel could be made available for Friday Prayers and that sequential Prayer sessions could be held.

The next stage in the process is to secure funding. Laura is working to submit appropriate paperwork to the Endowment Committee for a significant contribution towards costs. Dialogue will also start with the University to discuss their financial contribution. It was noted that it

Item Subject Action may be useful to breakdown both the NHS Grampian staff and University staff usage of the Prayer room.

Maggie’s Centre

3.4 Laura reported that the Maggie’s Centre is now fully operational. A large number of open days were arranged and members of the Committee were encouraged to visit the centre if the opportunity arises. Laura met with Susan Sutherland, Centre Head, to discuss spiritual care support for patients. Mark is meeting with Susan on 12th November 2013 to discuss a creative and effective way to provide spiritual support to the centre.

Vacancies at ARI & Woodend

3.5 Mark reported that Mairearad Ros has been appointed to the vacant chaplaincy post at ARI, hopefully starting in December 2013. She is currently the Chaplain at the Marie Curie Hospice in Newcastle upon Tyne

There are also two half time vacancies being advertised at present. They are stand alone posts but could be combined to provide a full time post. One is for lead role in Woodend Hospital. The other will be based at ARI, but with a view to deploying this person to community settings in Aberdeen City e.g. the Community Health and Care Village, Chaplaincy Listening in GP Practices and the Links Unit. The closing date is Wednesday 13th November 2013 with interviews being conducted at the end of November 2013.

The Fraserburgh post has now been filled. Rev Carol Ann Parker, a local Church of Scotland Minister, has been appointed to this post.

4. ARI Bereavement Centre

Jim reported that Sandra Stankus was invited to attend this meeting to discuss her work around the ARI Bereavement Centre. However, Sandra was unable to attend, and has been invited to attend the meeting in February 2013. A project description, achievements to date and future activities report was prepared by Sandra, which was circulated with the papers for information.

Some of the issues raised from the progress report:

- Spiritual Care did not seem to feature prominently on the agenda. This will be addressed with Sandra and her management team in the future to ensure links with the Spiritual Care Team. - The function of the Bereavement Centre still requires further clarification., - Liz Tait met with Sandra to discuss policies, accessing death certificates at ARI and volunteers working in the Bereavement Centre. - This is a six month post with a challenging and huge agenda to deliver. - Issues were raised as to why the Endowment Committee postponed a decision on the proposal for the Bereavement Centre (East Gate Lodge) from their September meeting to the December 2013 meeting. Laura assured the group that the issue would be discussed further at the December 2013 meeting.

A lengthly discussion took place around the Medical Certificates of Cause of Death (MCCDs). Revised proposals are to be introduced and are currently out for consultation.

Liz explained that the as part of the new procedure, the Proculator Fiscal will be required to look at 25% of death certificates and out of these a certain percentage will require to be submitted to an in-depth review panel.

Some of the concerns raised were:

- This will have the potential to cause lengthy delays in releasing bodies to families, concerning which a great number of complaints are already received by the NHS Grampian Feedback Team.

- The public are not aware of the new MCCDs being

Item Subject Action introduced and the implications this will have on families.

- The issue of how the Procurator Fiscal will receive the certificates and where the in-depth review panel would be situated was raised, as medical reviews do not sit within ARI. - It was highlighted that only one GP signature would now be required for cremation documents instead of two at present. - Liz reported that GPs were given the opportunity to make comments through Professor Sir Lewis Ritchie, Director of Public Health, who sent the consultation documents to the Chair of the GP for discussion at one of their meetings.

Laura has concerns around how people can influence this new procedure and that members of this group should take back this proposal to other committees they are involved in for comment. Concerns around this issue may also be required to be raised with MPs. It was agreed Liz would send Diane all the information she had around the MCCDs for circulation to the group.

It was agreed that a more detailed discussion would take place around this issue at the next meeting in February 2013, once the group has been able to digest the new procedure. Laura and Liz agreed to discuss this.

5. Chaplains’ Study Day – 9th October 2013

Mark updated the group on the recent Chaplains’ study day which was held on 9th October 2013. Around 50 people attended which consisted of Chaplains, members of other faith groups and volunteers. The morning session was a lecture on Living Well and Dying Well by Dr Sally Lawson which received very positive feedback. Stuart Hannabuss commented that the event was very well structured and interactive. Stuart also initiated the importance of involving volunteers in these types of events and to have links between research and practice.

6. SeSector Reports

6.1 Aberdeenshire – Morag Mitchell The following was noted:

- Glen O Dee is without a Chaplain at present due to personal reasons. Cover is being sought.. - Jim Simpson recently attending an Area Managers meeting at Inverurie and explained the role of the Spiritual Care Committee. The discussion was very informative and it was suggested Jim could attend one of the Senior Charge Nurse Forums in the near future.

6.2 Mental Health – Jim Simpson The following was noted:

- The management representation may change from Cornhill - it may not be Alistair Walker in the future. - Research work has been undertaken with Forensic Services and positive steps have been made to gain better access to spiritual care. - Being active in the Scottish Patient Safety Programme for Mental Health has given spiritual care more opportunity to engage. 6.3 Moray – Liz Tait The following was noted:

- Work is still ongoing around producing a procedure and policy document around disposal of ashes from babies. A Lead Nurse has been identified to get this work completed. This will be an agenda item at a future meeting of this group. - Liz and Mark have met to discuss succession planning in Moray. This is a good opportunity for redesign of posts in the future. This will be discussed again during January 2014. Dr Gray’s have been approached for bereavement care work to be added to the work pastoral care team. This would equate to around 7.5 hours.

6.4 Acute – Yvonne Wright The following was noted:

- There is concern around the fact the Chaplains are unable to access the PMS system due to data protection issues and are therefore unable to locate patients from Orkney and Shetland. A Lead Nurse has made Jonathan Lofthouse, Acute General Manager aware of the situation. Sharon confirmed that Chaplains are employees of NHS Grampian and if access to PMS is a requirement of their role and not made available then the staff governance standards are being contravened.. Laura agreed to have another look at this situation and feedback in February 2014. - There is still concern among the Chaplains that not all patients are being given the opportunity of a visit of the Chaplain. Mark confirmed the new nursing documentation is being implemented in all wards within the Acute sector as of 1st October 2013 but it is too early to say if this will help communication around this issue. Mark has written to all Ward Managers to have discussion around section 12 of the document and to meet with Ward Manager to discuss the importance of spiritual care. This will be an ongoing rolling process for Mark. It was queried whether this dialogue and meetings will be cascaded to Community Hospitals. The new documentation is already being used in Aberdeenshire Community Hospitals and one ward is using it in Moray. Mark agreed to look at sharing the dialogue across all other wards in NHS Grampian. Liz was happy to help to organise this for Moray. A review of the new documentation will take place NHS Grampian-wide in six months. - Mark has presented his programme on the SHMU Health Radio Show and discussed the role of the hospital Chaplains and spiritual care whilst in hospital. The show received a positive response. - Serena Stirling, Practice Education Lead for health Care Support Workers, has made contact with the chaplains with a view to doing some value based reflective practice work with this group of staff during their induction period. 6.5 City CHP – No representation from Aberdeen City and no update report submitted

7. AOCB

Discussion took place around future topics, agenda items, aspects of business for future Spiritual Care Committee meetings. It was agreed if any members have any ideas, then please email Diane Murray on [email protected] and Diane will then pass the ideas on to Laura and Jim.

It was noted that spiritual care and bereavement compassion is a function that many staff have problems understanding. Staff who require training/help and support on how to deal with these delicate issues.

Sharon Duncan explained that exploratory work is being undertaken which would enable staff to be able to document and share their experiences of their working day on another module within the Datix system; It means staff could reflect, share and record their experiences at work. This could cover any positive/negative incidents which occur and deemed outwith their job description.

At NHS Grampian Board meetings Patient Stories are used which are fed through the Feedback service; it could be an option that this new module could be used aswell as the patient stories. Laura noted that we need to think how to bring patients / staff stories to the Spiritual Care Committee.

Leanne Will reported she is currently leading on an MSc Professional Practice Project around spiritual care for nurses at RGU. This project covers the softer side of nursing. Once the project is completed, Leanne will present her findings to the Spiritual Care team, probably in November 2014.

It was agreed that Jim should invite Ewan Kelly to attend a future meeting to update the Committee around the agenda changes currently being implemented from NHS Education from Scotland.
