Gippsland Health Jeanette Douglas, Discuss Live demonstration 20 Pathways Launch Gippsland on how to log in and mins Primary Health use the site Network Jeanette demonstrated how to use the Gippsland Health Pathways website - gippsland.healthpathways.org.au/  Web based platform for Health Professionals in the Gippsland catchment  This was launched on the 1st February  41 pathways currently in the system  Contains a mixture of clinical pathways, referral pathways for local services and resource pages  Key stakeholders, GP’s and specialists are contributing to a working group to identify any system issues  Login is required to use the system. Access is requested via the home page - gippsland.healthpathways.org.au/  Currently working on a new service directory format within health pathways  You will find a form on the website to request a demonstration to your organisation

Outcome: Network members have a greater understanding how the Gippsland HealthPathways website works. Actions: An update on the Gippsland Health Pathways will be provided at each Complex Care network meeting.

Hazelwood Mine Fire: Ian Needham – Inform - 10 Latrobe Health Innovation Latrobe Health mins Zone. What are the ICDM Innovation Zone and Mental Health Forums? Ian Needham provided the following update on Latrobe Health Innovation Zone:  The Latrobe Health Innovation Zone (LHIZ) was established in response to the Hazelwood Mine Fire Inquiry  From the inquiry, reports were developed outlining recommendations, including the establishment of a LHIZ  One of the key components of the LHIZ was the establishment of a Latrobe Health Assembly (LHA). The aim of the LHA is for the community to have more say and to be able to take more ownership of what the general health issues of the region are  The LHA have met twice and currently has 41 members, with the intention of having up to 50 members. A majority of these members are community representatives. The other proportion is made up of direct invitations from government agencies and health organisations  The intention is to have a very strong community lead focus  The LHA will be reviewed in 18 months with the intent of improving the general health status of Latrobe Valley  LHA communication will be shared via the LHA members distributing the information  The LHA will look at how to add to the work that is already being done  Any thoughts, concerns or feedback (i.e. clinical feedback regarding what the gaps in the system) can be emailed to [email protected]

Outcome: Network members have a greater understanding the Latrobe Health Innovation Zone and Latrobe Health Assembly. Actions: An update on the LHIZ and LHA will be provided at each Complex Care network meeting.

Gay and Lesbian Health Victoria are providing 2 sessions in Gippsland, commencing tomorrow in Warragul although is fully booked.

CWGPCP will hold another Mental Health First Aid course in 2017. Date to be confirmed.

Prevention of Chronic Diseases - Healthy Ageing Seminar – Friday 24th March, Melbourne.

2017 Annual ADMA National Conference – Monday 11th and Tuesday 12th of September, Darwin.

Community Based Social Marketing Workshop - Monday 20th to Thursday 23rd March, Melbourne.

Diana Mueller (Dept of Education and Training)  NDIS implementation team is guiding Dept of Education and Training. Changes will occur to specialist’s children’s services

Tania Ryan (Gippsland Sectoral Development Team)  Changes will occur to Home Care Packages, Aged and Disability services via NDIS in Gippsland. Currently trying to work and advocate for people with diverse and challenging backgrounds  A Dementia Implementation group meeting will be held this afternoon to discuss the new Gippsland Dementia Plan implement and how to promote this regionally  Tanya presented a care journal which has been developed by another region. Options were discussed regarding developing a local journal and will be considered further at the next meeting

Bosede Adetifa (LRH)  Aged Mental Health Service has a nurse practitioner candidate (Grace Edgar) who will be running a “Nurse led clinic” to ensure physical health assessment is attended  It is hoped that this clinic will be fully operational within the next 3 months and teleconferencing opportunity will be considered  Current model of service is undergoing a re-design region wide  Acute Community Intervention Service is operational at LRH and supportive of Aged Team for after hours interventions

Suzanne Lewis (Latrobe City)  Direct care coordination is going through changes and currently implementing ways to help the community with the changes  Currently advertising for a Dementia and Access Support person

Karyn Thomas (LCHS)  Karyn is in a new role which includes a Dementia Falls portfolio with her primary role dealing with complex clients  Mine fire funding will be used to look at how to get the vulnerable communities

Alex Todorovski (Lifeline Gippsland)  Currently in the Business Operations Services role concentrating on increasing the volunteer base at Lifeline Gippsland for retail, administration and driving. All enquiries to 5136 3500

Marie Charleston (LRH)  Commencing Enhanced Maternity Clinic the week beginning February 20th  CDAMS have increased their staff by 0.2. Currently looking at getting the Bairnsdale clinic up and running  A nurse practitioner candidate commenced with Residential In Reach

Nancy Oosterhof (Baw Baw Shire)  Baw Baw Shire will soon be implementing a new client management system for clients under 65. Over 65’s will remain in My Aged Care  Reviewing the Terms of Reference for West Gippsland Coordinated Care meetings  It is still undecided if Baw Baw Shire will be a registered NDIS provider  Developing protocols and training for intake staff and exploring the option of joint visits  A part time Intake Officer has been advertised

Sue Smith (WGHG)  Complex care has expanded with 5 more beds in GEM at Neerim South  Residential in-reach is trying to get increased EFT to keep up with the demand  TCP numbers are down  Recently made the news as State and Liberal governments have backed the new hospital. The draft hospital drawings have been developed

Anthea Clarke (Latrobe Health Innovation Zone)  Thank you for inviting LVHA to this meeting Jeanette Douglas (GPHN)  Main focus is the launch of the Gippsland Health Pathways and getting to stakeholders

Liz Meggetto (CWGPCP)  Jess Chambers has extended her maternity leave with the CWGPCP until June. Have advertised and will backfill for the next 4 months. Liz will cover Health Literacy until Jess returns
