This form and all original receipts must be received by us to the address below no later than Friday, April 28, 2017. We are not able to accept reimbursement request after this date. Please allow 4-6 weeks for your reimbursement check to arrive.

Directions and Required Documentation After March 31, 2017, when you have completed your travel to the Duke MPP Program Admitted Student visitation day, please fill out the reimbursement form and provide necessary expense documentation. Due to University regulations, we cannot provide advance payment. Attach any original receipts (keep a Xerox copy for your records). Original receipts can include:

 vehicle fuel

 rental car receipts

 airline receipts with boarding passes

 parking receipts

 hotel room receipts

 mileage with address of departure listed Our reimbursement is limited to not more than $100 or the lesser amount if the receipts total less than $100. This form and all original receipts must be received by us to the address below, no later than Friday, April 28, 2017. Please allow 6-8 weeks for your reimbursement check to arrive.
