I consent to Manchester City Council passing the information on this form to WorkSolutions, Pinnacle People or Pertemps so that I can participate in the ESF project and receive support from WorkSolutions, Pinnacle People or Pertemps and the other organisations they work with. Background information and privacy notice

The information below explains:

 the purpose of this project;  why the family should have been contacted to see if they would like to participate; and  what will happen if they consent to being referred into the project by Manchester City Council.

The purpose of the project

European Social Fund (ESF) Support for Families with Multiple Problems aims to tackle worklessness by progressing families with multiple problems closer to employment. The provision will take a whole family approach, supporting individual family members across the generations. Participation on the programme is voluntary.

How they will benefit from the project

Manchester City Council works with large numbers of families which need support and assistance at various times for different reasons. The DWP (Department for Work and Pensions) has asked the Council to identify families it already works with which could benefit from some help getting back to, or ready for, work if one or more adults in the family is unemployed. The project is designed to provide families with extra support alongside support they may already receive from the Council and other organisations.

You will have asked if they would like to participate in this programme because either no- one in their family is working or there is a history of unemployment across generations of their family. They have also been asked because at least one member of their family is in receipt of a DWP working age benefit, although the person receiving this benefit does not actually have to participate in the project. Any members of the family who are currently working cannot participate in the project.

Members of the family who choose to participate in this project must be aged 16 or over but there is no upper age limit.

What will happen if you consent to being referred into the project by Manchester City Council?

If you sign the consent form and agree to participate in the project, Manchester City Council will send this referral form to Work Solutions, Pinnacle People or Pertemps. These three organisations are working on behalf of a company called G4S, which is working for the DWP to deliver services on the DWP’s behalf to help you gain employment. The City Council is acting as a referrer only; these organisations are not working on behalf of Manchester City Council. ESF FAMILY SUPPORT PROGRAMME

If you consent to participate in the project, the Council will send your completed form to Work Solutions, Pinnacle People or Pertemps. One of these organisations will then contact you directly to arrange a visit and collect more detailed information from you to assess the support they can provide to you to help you gain employment. Participation in the project is voluntary and you can withdraw at any time.

The information these organisations are likely to collect from you will include:

 Name and contact details  National Insurance Number  Date of birth  Ethnicity Information  Disability details  Employment Status  Details of family members  Any other information they need in order to provide services to you

Once you agree to participate in the project, Work Solutions, Pinnacle People or Pertemps may share your information with other organisations they work with in order to provide services to you; however they will explain this to you when they contact you directly. They will also pass your information to the DWP and Job Centre plus because they are working on their behalf. They are not working for Manchester City Council.

If you give information to Work Solutions, Pinnacle People or Pertemps, your information will be used by these organisations to:

 Register you and/or members of your family as a participant in the project  Research how well European Social Fund programmes work  Link with other information the DWP holds about you, if you are in receipt of a DWP working age benefit  It may also be used for any other DWP purpose and may also be passed to other government departments where this is permitted by law.

You will be provided with more information about this by Work Solutions, Pinnacle People or Pertemps when they contact you about the project. Participation is voluntary and you can still decide not to participate once you have been referred to Work Solutions, Pinnacle People or Pertemps.
