New Hire Enrollment

NEW HIRE ENROLLMENT

All full-time employees are eligible to enroll in benefits within 30 days from date of hire for benefits to be effective the first of the month following 30 days of employment. Employees must enroll on-line through Employee Space, no later than 30 calendar days from date of hire. If an employee does not enroll within 30 calendar days, you will automatically be enrolled in the Cigna CDHP medical plan. You will not be able make any changes to this option until the next open enrollment period unless you experience a qualifying event during the year.

Full-time and part-time employees may waive medical insurance if they have medical coverage through another group health plan. Employees who waive medical insurance must show proof of other coverage upon request.

During the year , if you experience a change in status, i.e. marriage, divorce, change in jobs, loss of coverage, newborn, etc., you may makes changes by completing a benefits change form within 30 days of the qualifying event.

Our benefits plan year runs from July through June annually, with open enrollment typically in the May timeframe.

The Enrollment Process: 1. Use the Step-by-Step Instructions for New Hire Online Enrollment to assist you through the process. Be sure to go completely through the process (do not exit as your enrollment will not be complete), check each of your elections prior to continuing, and print out your confirmation at the end. If you find your confirmation differs from what you intended, please contact HR Connect at 720-423-3900 for assistance. 2. If covering dependents, please enter all spouse and dependent information (including Social Security Numbers) in “personal information” prior to going through the online enrollment process. 3. Use the Benefits Guide and the Plan Selector Tool to assist you in selecting your benefits. When ready, log into Employee Space. Since you can only go through the enrollment process once, it’s helpful to have your plan selections made before logging into Employee Space. 4. To ensure that your elections were processed properly, check your first payroll stub through Employee Space and report any discrepancies to HR Connect, immediately upon discovery.

You will be offered enrollment in the following DPS insurance (if eligible): On line enrollment through Employee Space:  Health Insurance – medical, dental, vision  Health Savings Account (must enroll in a Consumer Driven Health Plan to be eligible to enroll in HSA)  Healthcare and Dependent Flexible Spending Accounts Paper enrollment ONLY (Included in your New Hire packet):  Sick Leave Bank – orange enrollment card. Please find the form in your new hire orientation packet and return the completed form to HR Connect.  Optional Life Insurance (DPS offered only, see Benefits Guide for instructions on enrolling in the PERA offered optional life). You are automatically enrolled in the DPS paid MetLife Group Life Insurance. The paper enrollment form offers you an opportunity to purchase additional life insurance for yourself, your spouse and dependent children. You must purchase coverage for yourself to apply for coverage for your dependents. If electing coverage for yourself over $100,000 and/or your spouse over $20,000, you will also need to complete the paper Evidence of Insurability (EOI) Form. Please find the form in your new hire orientation packet and return the completed form to HR Connect.

Important Information:  Effective Dates – upon completion of enrollment: Benefit Effective Date Medical, Dental and Vision First of the month following 30 days of employment. Sick Leave Bank Membership requires employees to donate the required number of personal accrued sick leave hours the first of the month following 60 days of employment. SLB effective date coincides with this deduction process.

 Declining Medical Insurance: If you decline coverage (waive) medical insurance you must send proof of other insurance coverage (copy of card, letterhead communication from health plan or spouse’s employer, etc.) to HR Connect, Admin Bldg., Suite 105, Fax 720-423-2505.  Record your beneficiaries: Please see the Benefits Guide for instructions on designating your MetLife insurance beneficiaries.  Voluntary Insurance: If you wish to enroll in any voluntary benefits or the additional retirement options (403(b) or 457(b)), please first contact the appropriate carrier representative to begin the sign-up and enrollment process. If you wish to enroll in the 401(k), just complete the Plan Contribution Authorization form and fax it to HR Connect at 720-423-2505. These elections may be changed, dropped or added at any time during the year and need not wait for the open enrollment period. OPEN ENROLLMENT Open Enrollment is generally during the month of May; watch for communications every April. If you enroll in the health care and/or dependent care flexible spending account you must re-enroll during the open enrollment period to continue these accounts. Sick Leave Bank Open Enrollment for new membership is the entire month of September with the required day(s) deducted in November and the effective date of new membership Dec. 1. The purpose of the Sick Leave Bank is to provide the means of obtaining additional sick leave days to eligible employees who are on an approved extended personal illness leave, have exhausted all of their accumulated leave and would otherwise be on an unpaid leave status.
