Statement of Policy regarding fitness to practise proceedings by a licensing/regulatory body and relating to criminal investigations in the UK or overseas. Registration with the General Medical Council or General Dental Council imposes on doctors and dentists the duty to provide a good standard of medical care for, and to behave appropriately, towards patients. NHS Employers also have a duty to ensure that patients receive a good standard of medical care and ensure as far as possible the safety of patients. We therefore need to establish if you have been found guilty of a criminal offence, been bound over or cautioned or are currently the subject of proceedings which might lead to a conviction, an order binding you over or a caution, in the UK or any other country.

CLINICAL FELLOW IN GENERAL OR VASCULAR/ENDOVASCULAR SURGERY FOR ONE YEAR

Applications are invited for a Clinical Fellow at ST6 or above (or equivalent) in general/vascular surgery. This post, is funded by NHS Tayside and is a one year fellowship designed for surgical trainees towards the end of their surgical training (ST6 or above (or equivalent) who wish to concentrate on an area of specialist training. Applications are also invited from candidates who have recently obtained their CCT and who wish a period of advanced training in general or vascular surgery. Interested candidates are encouraged to discuss their specialist training requirements in advance. The potential specialties available are minimal access upper GI/HPB surgery, minimal access colorectal surgery, endocrine surgery, breast surgery and vascular /endovascular surgery. The fellowship will be based in one or more of these specialties depending on the preference of the successful candidate. The Vascular Unit is Vascular Society/Cook Endovascular approved for fellowships. The Colorectal Unit has a separate pelvic floor fellowship post. Full participation in the general surgery on-call rota is required. The combination of general and vascular surgery may be of particular interest to military trainees and those interested in trauma surgery. TAYSIDE ROTATION

This post, is funded by NHS Tayside and is a one year fellowship designed for surgical trainees towards the end of their surgical training (ST6 or above (or equivalent) who wish to concentrate on an area of specialist training. TAYSIDE AND DUNDEE Tayside Region encompasses the former counties of Angus, Perth & Kinross together with the Cities of Dundee and Perth, and the Burghs of Arbroath, Forfar, Brechin and Montrose. Dundee is Scotland’s fourth largest city and is located on the northern bank of the River Tay. The city provides a mixture of traditional and modern living and the surrounding area provides an extensive choice of excellent housing and schooling. Most leisure activities are available close by with particularly good facilities for hill-walking, climbing, skiing and golf.

THE HOSPITAL Ninewells Hospital and Medical School is a large general teaching hospital and provides a full range of services with specialist units with the exception of transplantation and cardiothoracic surgery. The patients derive from a mixture of rural and urban areas and total a population of 450,000 to 750,000 for the various regional services the hospital provides. The surgical service has a particularly high profile as the home of the internationally renowned Cuschieri Surgical Skills Centre and for being in the forefront of the development of minimal access surgery.

Ninewells Hospital has been designated one of the five Cancer Centres in Scotland and provides, on a single site, all general surgical services, regional Radiotherapy and Medical Oncology. These Oncology Services complement the international standing of laboratory biomedical research. The Surgical Service The General Surgical Service includes emergency general surgery, vascular surgery, upper gastro-intestinal and hepato-biliary surgery, breast surgery, colorectal surgery, endocrine surgery and paediatric surgery. Wards consist of 24 or 30 beds and are currently arranged so that elective surgery is carried out from four wards. There are currently 10 high dependency unit beds within the Directorate, an acute surgical receiving unit (ASRU), and a Theatre Admission Suite with two theatres. There is also an 8-bedded Intensive Care Unit within the Anaesthetic Directorate. Each of the 2 general surgical units is on-call on a one-in-two basis. During a unit’s on-call week its elective activity is curtailed. General Surgery serves a population of up to 450,000 including all regional trauma; urology and head injuries are not part of the general surgical workload. The separate vascular service is provided by a network between Tayside and Fife (population total 750,000) with all emergencies received at Ninewells Hospital. Elective vascular surgery is performed in both Ninewells Hospital and Victoria Hospital, Kirkaldy. Further daytime general surgical services are provided at Perth Royal Infirmary and Stracathro Hospitals.

Clinical Duties The successful candidate will work within Ninewells Hospital / NHS Tayside at the level of a ST6 or above. The specialties included are minimal access upper GI/HPB surgery, minimal access colorectal surgery, endocrine surgery, breast surgery and vascular surgery. The fellowship will be based in one or more of these specialties depending on the preference of the successful candidate. The training provided by this fellowship will be overseen by Mr. KL Campbell General Surgery Training Programme Director for the East of Scotland Deanery or by Mr John Nagy Consultant Vascular Surgeon and Clinical Director - Surgery. Full participation in the general surgical on-call rota will be required.

Upper GI / Hepatobiliary Surgery The Upper GI/HPB unit founded by Professor Sir Alfred Cuschieri has a strong pedigree in Laparoscopic and Minimal Access Surgery and is able to offer fellowship training in Intermediate & Advanced laparoscopic surgery. The team has considerable expertise in all aspects of open and minimally invasive abdominal and visceral surgery including benign and malignant oesophago-gastric, hepatobiliary, pancreatic, bariatric, HIPEC and general surgery. Training is a major interest and regular laparoscopic courses in upper GI surgery and bariatric are run by the team in the Cuschieri Skills Centre.

Colorectal (CR) Surgery The unit is a leading centre in laparoscopic colorectal resection. The CR service includes Transanal Endoscopic Microsurgery, Cylinder APR, Anorectal Physiology, Transanal Ultrasound, the Functional Bowel Disorders Service, and Sacral Nerve Neuromodulation. The unit is recognized by the Pelvic Floor Society for training. The CR unit has now expanded to become active at the Perth Royal Infirmary site in addition to the Ninewells site.

Endocrine Surgery

Ninewells Hospital can provide training in the largest surgical endocrine practice in Scotland (60 thyroids, 80 parathyroids and 15 adrenals [open/laparoscopic] per year). There is a weekly clinic with interface with endocrine physicians and a combined USS-FNA clinic with radiology and cytopathology input. The Thyroid MDT involves collaboration with Grampian and Highland patients, and all aspects of thyroid cancer care is discussed. There is opportunity to undertake further endocrine work in Grampian in addition, with a combined catchment population for parathyroid surgery and adrenal surgery of over 1 million.

Vascular surgery The East of Scotland Vascular Network (ESVN) serves a population of 750,000 from Tayside and Fife. All emergency and all major elective vascular surgery are performed in Ninewells Hospital. In 2012 ESVN became an approved unit for the Vascular Society/Cook Endovascular Fellowships. ESVN has an advanced endovascular program treating thoracic syndromes and abdominal aneurysms with conventional and fenestrated stents in addition to peripheral arterial and venous interventions. The full range of open vascular surgery is practiced excluding open thoracic aneurysm repair. The combination vascular with general surgery is particularly suited to military trainees and those interested in trauma surgery.

Breast Surgery Across two sites, Ninewells Hospital and Perth Royal Infirmary, over 470 new breast cancers are treated annually through the symptomatic and screening services. All aspects of breast cancer surgery are provided including localisations for impalpable disease, and sentinel node biopsy. The breast team have joint service with the plastic surgery team for breast oncoplastic and reconstructive services. All referred patients are reviewed through a joint weekly oncoplastic surgery clinic for a range of techniques including therapeutic mammoplasty, immediate and delayed breast reconstruction including implant, pedicled and free flap procedures, lipomodelling and symmetrising surgery. There is a strong reputation for breast cancer research recruiting into local, national and international trials.

Emergency Surgery At the Ninewells site, 12 consultant general surgeons provide cover for emergency general surgery. Each unit is on call alternate weeks. There is 24-hour access to a staffed emergency theatre enabling most emergency surgical procedures to be dealt with promptly. Consultant input to the emergency workload is high maximising the teaching and training opportunities. Cover for vascular emergencies is provided by the 7 consultant vascular surgeons of the ESVN, supported by the general and vascular surgical middle grade staff.

THE UNIVERSITY The University of Dundee was founded in 1967, and the medical school dates from 1974. Before this, Dundee acted as a college of the ancient University of St Andrews, with which it retains strong links. The main campus is situated close to the centre of town, and the recent opening of the Wellcome Trust Biocentre and current building of a centre for Diabetes Research testifies to the University’s pre-eminence in biochemistry. The Medical School, housed in Ninewells Hospital, also has an exceptional research and teaching track record.

Research A commitment to research and a proven track record is considered useful. The successful candidate will be encouraged to participate in clinical or laboratory based research related to their field of interest.

The Academic Department Of Surgery The University Department of Surgery is based at Ninewells Hospital, is supported by government (MRC and Chief Scientists Office for Scotland) and major charity (Cancer Research UK) funding. Clinical academic staff include Professor RJC Steele (Head of Department, main interest Colorectal Cancer). There are also several honorary members of the Academic Department including NHS consultants in Surgery, Oncology and Palliative Care.

The Department is also interested in techniques of training in surgery and developing ergonomically sound surgical technique. For many years there has been a very active Surgical Technology Group under the directorship of Professor Sir Alfred Cuschieri. This has moved to a new Institute of Advanced Medical Technology (IMSAT), distinct from the Department although it retains strong links with IMSAT.

The Institute for Cardiovascular Research (TICR) The Institute brings a multidisciplinary team (physicians and surgeons - cardiology, stroke medicine, heart failure hypertension, vascular medicine and vascular surgery) together whose focus is cardiovascular disease. There are a number of units within the group (Peripheral Vascular Diseases Research, Cardiovascular Epidemiology, Cardiovascular Medicine, Cardiovascular Pharmacology, Public Health Nutrition Research, and The Angiogenesis Unit). The Institute also is in partnership with other medical and non-medical academic units within the University. The academic leadership within Peripheral Vascular Disease comes from Professor Peter Stonebridge (interests include blood flow dynamics, vascular service development and quality initiatives within the Health Service) and Professor Jill Belch (interests in vascular risk reduction and microvascular disease).

Clinical Research Centre A Clinical Research Centre for trials and clinical studies has been funded and will be built over the next 18 months to facilitate continued success in patient studies and clinical trials to the required standards of European law.

Teaching Undergraduate and postgraduate teaching are considered of the utmost importance, with many components of the undergraduate course organised from the Departments. Undergraduate teaching includes students from year 2-5 of the 5 year MBChB course, and teaching is usually seminar and small group based with students able to undertake Student Selected Components in clinical or laboratory aspects of surgery.

For postgraduates, there are strong links to the Cuschieri Surgical Skills Unit and to teaching on courses for the Royal College of Surgeons of Edinburgh. These include courses in anatomy, basic surgical skills, endoscopic skills and laparoscopic skills at all levels.

DUTY ARRANGEMENTS The rota is a full shift system. The hours of duty will be the standard working week of 48 hours.

This is a one year Fixed Term post commencing 1st October 2016 (subject to negotiation)

When the post holder is requested to provide cover on a locum basis outside his/her own job description in circumstances other than emergencies and unforeseen circumstances, the Trust will contract with him/her for that purpose at the standard rate in accordance with the Conditions of Service.

Emergencies & Unforeseen Circumstances The junior doctor accepts that he/she will also perform duties in occasional emergencies and unforeseen circumstances, at the request of the appropriate Consultant in consultation where practicable, with his/her colleagues both senior and junior. It has been agreed between the professions and the Department, that while juniors accept that they will perform such duties, it is stressed that additional commitments arising under this sub-section are exceptional and in particular that juniors should not be required to undertake work of this kind for prolonged periods or on a regular basis.

It is the responsibility of the Consultant(s) to determine whether a particular situation can be described as an emergency.
