Lessons Learned about Retention in Care

Help us to collect best practices to retain patients in HIV care. II. Retention Tools If your organization uses standard documents, tools, forms, checklists, call scripts, charts, instructions or other relevant items, please attach to the email and identify them below. Please provide a brief description of how each tool is used.

Retention Measures If your organization has utilized measures for retention, please explain the parameters below next to Measure 1, and 2, as needed.  For Numerator please explain what your numerator represents and how you collected that information  For Denominator please explain what your denominator represents and how you collected that information. .  For Recent Results please list the most recent measure results. (These will not be shared.).  For Lessons Learned, explain what you have learned about the measure, your organization, and your consumers. This is your opportunity to give us some concise feedback on both your successes and the lessons you learned.

To help you describe your retention measures, an example of a HAB measure definition is below:

Example of Measure: HAB CD4 T-Cell Count Measure

Percentage of clients with HIV infection who had 2 or more CD4 T-cell counts performed in the measurement year

Numerator: Number of HIV-infected clients who had 2 or more CD4 T-cell counts performed at least 3 months apart during the measurement year Denominator: Number of HIV-infected clients who had a medical visit with a provider with prescribing privileges1, i.e. MD, PA, NP at least once in the measurement year Patient Exclusions: 1. Patients newly enrolled in care during last six months of the year

Data Element: 2. Is the client HIV-infected? (Y/N)

3. If yes, did the client have a CD4 count test conducted during the reporting period? (Y/N)

a. If yes, list the quarters of these tests

Data Sources: • Electronic Medical Record/Electronic Health Record

• CAREWare, Lab Tracker, or other electronic data base

• HIVQUAL reports on this measure for grantee under review

• Medical record data abstraction by grantee of a sample of records
