ACCESS TO CARE AT KINGSWAY PHYSICAL THERAPY L.L.C:

We accept most major health insurances including MEDICARE, Anthem Blue Cross/Blue Shield and TRICARE.

- If you do not have health insurance, we are able to provide you with the care you need for a fee.

- Do not hesitate to call us with questions/concerns about insurance coverage/payment issues.

To access Kingsway PT for rehabilitative care:

- Obtain a prescription for Physical Therapy from your physician.

- Call us at (757) 325-8252 to schedule an appointment for initial evaluation by our Physical Therapist. Subsequent treatment visits will be planned based on the findings obtained after your initial evaluation and scheduled on your first day.

- We have various appointment times to accommodate your schedule including same day and after hours appointments. We are committed to getting you the care you need promptly.

- If you cannot keep your scheduled appointment, and to help us to ensure we provide prompt/timely care to all our patients, please call us at least 24 hours before your scheduled appointment to reschedule. We will do our best to find you another appointment that will be good for you.

- We will need information about and your insurance at the time you call for initial evaluation.

- If you have access to the internet, go to our website, print and complete patient forms and bring them with you to your first visit. If you do not have access to the internet, we will have you complete the forms when you arrive for your first visit.
