Healthcare Scientist Associate Practitioner in Biomedical Science

An opportunity has arisen for a Healthcare Scientist Associate Practitioner to join the Pathology department based at Forth Valley Royal Hospital in Larbert for a 1 year fixed term/secondment contract.

The successful applicant must have a relevant qualification at SCQF Level 8 (e.g. HND in Biomedical Science or equivalent) and previous experience of working in a Histology laboratory with skills in microtomy of tissue blocks is desirable.

Depending on the demands of the service, you will be employed to work as required over 7 days which include public holidays, and weekends. This is a busy, well equipped department providing Pathology and Mortuary support to the Acute Hospital, Community Hospitals and General Practice in Forth Valley.

1. JOB DESCRIPTION

2. JOB PURPOSE

 Process a broad range of laboratory diagnostic investigations in Pathology using a range of scientific measurements and technical procedures on patient samples.  Analyses and is responsible for own work and operates complex equipment.  Provides restricted advice to clinicians as appropriateness of tests and timescales.  Acts as a mentor to less experienced HCS, Support Workers and trainee HCS.

The Clinical Laboratories in NHS Forth Valley (NHSFV) comprises Biochemistry, Haematology (including Blood Transfusion), Microbiology (including Virology) and Pathology (including Cytology and Mortuary) Services.

A comprehensive, analytical, interpretive and clinical advisory laboratory based service is provided to:

 Hospital In-patient, Out-patient and Day Care Units.  General Practitioners in Primary Care and Community Health Services and Hospitals.  Public Health Medicine.  Occupational Health  Procurators Fiscal at Stirling, Falkirk and Alloa

The Laboratories support point of care testing directly in wards and clinics and at local Health Centres e.g. one stop breast clinics, Anticoagulant clinics, Genito Urinary Medicine, Male infertility clinics, diagnosis of Cystic Fibrosis,

In addition the services collaborate in a range of research and development and clinical audit projects with clinicians and other Healthcare professionals and provide research and development support to drug and diagnostic companies.

A staff of 180 provide services to a Forth Valley population of 265,000 with a total of more than 5 million investigations per annum ( > 1.2 million samples). Approximately 50% of the investigations are carried out for hospital based patients and 50% for the 210 General Practitioners in around 56 locations. Population screening programmes include Cervical Cytology, Blood Borne Virus Testing (e.g. antenatal screening) and other investigations link to programmes of health improvement across the county.

All acute aspects of the service operate 24 hours each day 365 days per year.

The service is provided by various staff including Healthcare scientists in Biomedical Science, Administrative and Clerical Staff, Cytology Screeners, HCS Support workers, Medical Staff, Mortuary Staff, Nursing Staff and Phlebotomy Staff. 5. KEY DUTIES / RESPONSIBILITIES

Responsible for providing a high standard of patient care: Frequently required to work on their own but with qualified staff generally available for advice.

Policies and Procedures  Adherence to policies and procedures relevant to all areas of work in accordance with Departmental, Directorate, Hospital and regulatory requirements. These would include the following: ― All Standard Operating Procedures ― Quality Management Policies ― External Quality Assessment and Internal Quality Control ― Laboratory and Hospital Health and Safety ― Risk Management ― NHSFV policies and current data protection legislation.

Professional and Scientific  Processes a range of laboratory diagnostic investigations using a range of scientific measurements and technical procedures on patient samples.  Validates clinical laboratory results and reports results to service users e.g. Clinicians.  Interprets results of routine tests.  Reports critical or urgent results directly to clinicians/nursing staff with referral to more senior staff/Consultant where required.  Prioritises own workload  Examples of routine laboratory procedures which are performed daily e.g. o Tissue embedding. o Tissue sectioning. of paraffin embedded blocks o Slide preparation and staining o Processing of non Gyn Cytology specimens. o Prepare solutions and reagents as per Departmental SOP’s  Ensures the continuity of the service is maintained by managing the efficient and effective use of expensive consumable materials.  Rotates through a number of routine sections of the laboratory.  Performs near patient testing.  Liaises with colleagues in other departments and hospitals to arrange specialised tests to be performed.  Performs delegated duties as agreed with the line manager.

Supervisory  Mentoring of Trainee HCS and Support workers working in that area

Quality Control and Quality management  Daily instrument testing, first line maintenance repair, and performance of quality control on a daily basis for equipment for own use and for other staff to operate.  Responsible for communicating QC failure or instrument malfunction to line manager. Research and Development  Participates in the evaluation and implementation of new equipment and analytical procedures.  Participates in the production of reports for internal and external audit .  Keeps up to date on the scientific, technical and theoretical developments in laboratory field.

Teaching/training  Acts as a mentor to less experienced HCS Support Workers, student placements and may train other staff on instruments or techniques where appropriate.

6. SYSTEMS AND EQUIPMENT

SYSTEMS  Ensures the integrity of the patient database within the laboratory computer system by accurate registration of patient demographics and request details.  Inputs laboratory results generated by self and others.  Uses Laboratory Information System to access patient information and validate laboratory results and reports  Utilises the electronic Quality Management System.  Queries the Laboratory Computer system for patient information.  Operates interfaces between laboratory Computer system and complex analytical equipment.  Uses proprietary software packages such as Microsoft Office.

EQUIPMENT  The post-holder is personally responsible for the safe use and maintenance of a range of highly complex equipment costing up to £100,000 for a specialised piece of equipment such as a multitest analyser, chromatographic equipment radioisotope counter and vacuum assisted tissue processor.  Performs first line maintenance on equipment to ensure the accuracy of results and the safety of all subsequent users.

7. ASSIGNMENT AND REVIEW OF WORK  Follows standard operating procedures (SOP’s) and may suggest amendments.  Follows all related directorate policies and procedures.  Operates in section of the department referring unexplained results to line manager.  Is competent in interpreting a range of data requiring analysis in a variety of formats and making valid judgements on the basis of data.  Validates clinical laboratory results, reports results to service users e.g. clinicians.  Deals with technical enquiries from trainee Healthcare Scientists, newly qualified staff and support staff 8. COMMUNICATION AND WORKING RELATIONSHIPS

 Provides explanation of laboratory information to laboratory staff, clinicians and other users  Receives information from laboratory staff, clinicians and other users varying from routine to complex requiring interpretation, analysis and further action.  Interacts with service users in stressful situations.  Notifies appropriate staff of clinically significant results or results outwith reference ranges.  Communicates important information regarding specimens, which may be critical to patient treatment e.g. urgent sample arrives, specimen unfixed  Deals with enquiries from all sources: When required directs enquiries to senior laboratory staff/Consultants.  Assists in the training/mentoring of less experienced HCS Support Workers and trainee HCS.  Participates in departmental meetings and contributes to effective communication within the department.  Communicates with other staff groups including Medical, Nursing, Clerical, Porters, Estates staff and Drivers.  Communicates non-conformities or malfunctions to senior Healthcare Staff.

9. DEMANDS OF THE JOB (PHYSICAL, MENTAL, EMOTIONAL)

Physical Skills  Frequent use of microscope or similar specialist devices  Accurate hand/eye co-ordination required for manipulation of sample material with forceps (some less than 1mm)  sectioning of wax blocks of tissue on microtome (some less than 1mm)  Manipulating 4 sections on to glass slides.

Physical Demands  Long periods of repetitive movements when cutting sections  Ongoing requirement for being in a restricted position e.g. embedding and sectioning  Some manual handling may be undertaken, such as manipulation of gas cylinders, moving of bulk reagents or clinical waste.

Mental Demands  The job demands frequent periods of prolonged concentration for the majority of a shift e.g. embedding and sectioning.  May involve entering and screening large quantities of numerical data, which requires intense concentration to interpret data.  Involved in maintaining a service in the presence of possible adverse events including equipment failure.  Job requires multi tasking. There are frequent interruptions from phone calls and technical queries causing occasional complete change of work priorities.  All work subject to deadlines.

Emotional Demands  Pressure to ensure clinical specimens are examined correctly and timeously, knowing that inaccurate and delayed results could result in inappropriate treatment of patients.  Indirect emotional stress from information found on biopsy and post mortem forms  Observation of recognisable body parts and foetal tissue on a daily basis.  Safe handling of dangerous materials e.g. HIV positive blood

Working Conditions  Continuous, unavoidable exposure to open samples of blood and other potentially infectious biological body fluids.  Manual manipulation of highly unpleasant specimens e.g. sampling from faeces specimens for culture, sampling of body tissues.  Continuous risk of exposure to or spillage of reagents, chemicals, solvents, during test analysis, many of which are potentially hazardous or carcinogenic.  Exposure to highly unpleasant smells from autoclaving of laboratory waste e.g. stool and urine samples.  Exposure to sharp instruments – microtome blades, forceps, probes  Exposure to hot molten wax

MOST CHALLENGING ASPECTS OF THE JOB  Mixing technical and clinical aspects of analysis to provide a safe, accurate and timely service, reducing the potential serious consequences of a laboratory error.  Making decisions about the clinical significance of laboratory results, taking into consideration; clinical specimen, the age and sex of patient and relevant clinical details.  Prioritisation of urgent requests including clinical emergencies.  Ensuring the accuracy of the work in a busy environment

10. KNOWLEDGE, TRAINING AND/OR EXPERIENCE TO DO THE JOB

Minimum of SCQF level 7 (e.g. HNC in Applied Science, SVQ level 3 in Laboratory and Associated Technical Activities or equivalent) with previous experience as a Healthcare Science senior support worker with a commitment to study and attain a qualification at SCQF level 8 (eg HND in Biomedical Science or equivalent qualification) within an agreed time period.

Staff taking up a new appointment with NHS Forth Valley will normally enter the scale at the minimum of the pay band/range. Protecting Vulnerable From 28th February 2011 the Scottish Government Groups Scheme introduced a new membership scheme to replace and (if applicable) improve upon the current disclosure arrangements for people who work with vulnerable groups. If the duties of this post involve regulated work with children and young people and/or protected adults, the successful candidate will be required to be a member of the PVG Scheme and consent to NHS Forth Valley obtaining a Scheme Record or a Scheme Record Update if they are already a member of the Scheme. Police Act If the duties of this post require the declaration and Disclosure Check: validation of criminal convictions, the successful (if applicable) candidate will be required to complete a standard or enhanced Police Act Disclosure Scotland check prior to any offer of appointment being made. In the event of employment, any failure to disclose such convictions could result in the withdrawal of any offer, disciplinary action or dismissal by NHS Forth Valley.

Health & Safety: All employees have a responsibility for their own health & safety and the health & safety of others who may be affected by what they do. Employees also have a duty to co-operate with their employer by following NHS Forth Valley policies and procedures and safe systems of work; by using equipment safely and by bringing any shortcomings in health and safety arrangements to the attention of their employer. Where something is provided in the interests of health & safety employees must not interfere or misuse it. All employees have a legal responsibility to report any shortcomings in terms of this in their area. Managers and supervisors have a responsibility for monitoring health & safety arrangements and ensuring staff are following policies and procedures and safe systems of work.
