HEARINGS - Mary Ellen Gillon:

1. VANARSDALE, ROBERT - LCA.6224 Commission continues formal hearing at request of Fire Marshal Smith.

2. BONGO, CHRISTOPHER - LIP.12827 Remove from hearings. Permit canceled.

3. KHAN, YASMEEN - LCA.6581 After compliance meeting, remove from hearings. No further action on incidents of October 7-8, 2004 and February 18, 2005.

4. KUDELSCHUK, ANTOINETTE - LIR.14207 After compliance meeting, commission accepts offer in compromise in lieu of hearing of 4 days and $1,000 for incident of 1/21/05.

5. CARR, DANIEL - LCA.6386 After compliance meeting, commission accepts offer in compromise in lieu of hearing of 1 day and $750 for incident of 8/7/05.

2. FONTANELLA, STEVEN - LCA.6760 Tradition Golf Restaurant, Wallingford Ok to issue provisional permit when in receipt of stipulated agreement, $500 fee, cancel permit LCA.6041, bill of sale, seller’s affidavit, executed copy of lease/deed, financial affidavit (source of $2,500,000. cash and loan documents for $2,500,000.), information release forms for D’Natale, LLC filing documents, FEIN, copy of sales tax permit.

3. WORTH, DOMINIQUE - LCT.165 The Corner, Bridgeport No requirements. Ok to issue final permit. Commission approved substitute permittee.

10. REYES, GILBERT - LGB.14076 Eva Maria Variety, Stamford Ok to issue final permit when in receipt of completion of application (section N), FEIN, cancel old permit LGB.12358, copy of sales tax permit, seller’s affidavit, proof of lockable storage, 8 x 10 photos of exterior.
