SEALINK INC. A ZERO TOLERANCE ORGANIZATION WILLIAM LUND MEMORIAL SCHOLARSHIP

FOR ADVANCED MARITIME AND EDUCATIONAL OPPORTUNITIES COLLEGE SCHOLARSHIP AND MARITIME VOCATIONAL TRAINING APPLICATION FILING. Applicants must be Ketchikan or Revilla High School graduates; Ketchikan Home Schooled or Ketchikan Private Schooled Graduates or GED recipients.

AWARD PROCESS: Applicant files containing the following will be considered for award: a) Introduction essay; indicating why you are requesting a scholarship and your career goal. b) Completed LUND scholarship application. c) Official high school, college transcripts or GED scores, whichever is most recent, indicating an overall 2.0 GPA or better, along with a copy of the High School Diploma and or GED. d) Provide a copy government issued picture I.D. (Drivers License, State I.D. etc.) e) Letter of Admission/Acceptance (LOA) from the school you are planning to attend. This LOA must indicate full or part-time or distance learning enrollment, type of maritime degree, maritime license or maritime certification to be attained upon completion of schooling/training f) Any other documentation/verification from Required Document List or as requested by the SEALINK office.

APPLICATION DEADLINE: For four year University scholarship applicants; COMPLETED APPLICATIONS MUST BE RECEIVED AND/OR POST-MARKED ON OR BEFORE June 15 FOR FALL SEMESTER AND December 1 FOR SPRING SEMESTER.

IT IS THE STUDENT’S RESPONSIBILITY TO TURN IN A COMPLETED APPLICATION BY THE DEADLINE OR RISK PLACEMENT ON THE WAIT-LIST. WAIT-LISTED STUDENTS RECEIVE FUNDING AS THE BUDGET ALLOWS.

FOR ALL OTHER SCHOLARSHIP APPLICANTS; UPGRADER AND PAUL APPLICANTS: APPLICATION MUST BE RECEIVED AND/OR POST-MARKED NO LATER THAN THIRTY DAYS PRIOR TO ATTENDING THE APPROVED TRAINING FACILITY AND COURSE OF STUDY.

WAIT LISTED APPLICANTS: Applications are considered incomplete until all documentation is on file with the SEALINK office. Complete applicants are put on a wait list pending award processing. Award considerations are based on the following: a) subject to the availability of funds at the conclusion of the scholarship awarding process; b) In order; according to date of receipt of completed application.

CHANGE IN ADDRESS/UPDATED INFORMATION: If applicant has ANY CHANGES after the submission of this application, it is the RESPONSIBILITY of the applicant to notify SEALINK as soon as possible. No contact will result in loss of award consideration and/or closure of file.

OFFICIAL STUDENT APPLICATION

The scholarship program provides financial aid to Ketchikan residents who have either graduated or received a GED diploma through the Ketchikan school system who are attending, or plan to attend, an accredited Zero Tolerance Maritime related college/university or Maritime related vocational training facility in the pursuit of undergraduate and graduate degree programs (post-secondary education). or a maritime license or certification.

Undergraduates in the Maritime related college/university must maintain a minimum 2.0 GPA or better each semester, to qualify for term funding.

Applicants to Vocational Maritime programs that are recent high school graduates that are considering a career through the Paul Hall Maritime Training Center in Piney Point MD will be funded to the school.

Applicants currently working in the maritime industry and are looking to upgrade their certification or attain higher license certification in the maritime field; any approved scholarship is based on reimbursement, upon successful completion of course work. (Applicants must attain a certification of completion or license in order to receive a scholarship reimbursement.)

STUDENT AGREEMENT To continue receiving funding through the William Lund Memorial Scholarship Program and to avoid probation and/or suspension I agree to comply with the following: INITIAL EACH BOX:

_ Be taking coursework that is applicable towards an maritime undergraduate or graduate degree; U.S. Coast Guard maritime license or certification.

_ Maintain a minimum semester by semester grade point average (GPA) of at least 2.0;

_ Submit an official grade transcript at the end of each academic term to; SEALINK

_ Provide government issued picture I.D.

_ Provide official class schedule with: student name, school name, courses, credits, and terms.

_ Provide 3 letters of recommendation from work supervisors, co-workers, school counselor former teachers etc.

_ Notify the SEALINK office of any changes in contact information such as: name change; mailing address; telephone; email; etc.

_ Notify the SEALINK office if you anticipate dropping below 12 credits for full-time and 6 credits for part-time;

_ Notify the SEALINK office if you anticipate dropping out of school;

_ Notify the SEALINK office if you anticipate changing to a different school;

_ Apply each year for federal financial aid (www.fafsa.ed.gov);

_ Complete and send to SEALINK an Advanced Educational Opportunities ‘update each year that you are enrolled in college classes;

_ Submit a copy of your official class schedule every semester showing; student name, school name, courses, credits, and term GPA;

I fully understand and have been made fully aware by SEALINK staff that should I fall below a 2.0 grade point average I will be placed on probation and funding will be suspended. I also know that if I do not show improvement, after one semester, I will be suspended from the program entirely and will not receive further Lund scholarship funding from SEALINK or any of its affiliates.

A student who experiences personal difficulties should notify the SEALINK staff and School Advisor, to make arrangements for maintaining good academic standing.
