DIOCESE of JOLIET CHAPTER KNIGHTS OF COLUMBUS Victor Cardosi Award Recognition Dinner 2015 Nomination Form
The Joliet Diocesan Chapter of the Knights of Columbus annually holds a Recognition Dinner, for those individuals selected to be recipients of the Victor Cardosi Award. The selected individuals are those who, as Knights, have exemplified Charity, Unity, Fraternity and Patriotism in Family, Community, Church, Youth and Council Activities above and beyond that of the average council member. Chapter Officers are exempt from receiving this award. The 2015 Victor Cardosi Award Recognition Dinner will be held on Saturday, April 18, 2015 at St. Mary Church, 19515 South 115th Avenue, Mokena, IL. Nomination RULES Nomination will only be accepted from the Grand Knight of a Council If Grand Knight is being nominated, than Deputy Grand Knight or Financial Secretary Nomination submissions should use a 12 point Times New Roman font. Nominations will be accepted using the following formats: 1. Microsoft Word documents 2. Adobe PDF files. 3. Printed documents mailed. CHAPTER DUES MUST BE PAID PRIOR TO OR NO LATER THAN THE OCTOBER 24, 2014 CHAPTER MEETING. CHAPTER DUES MUST HAVE BEEN PAID FOR THE PREVIOUS 2013-2014 FRATERNAL YEAR ALSO BY OCTOBER 24, 2014 All nominations must contain all eight (8) sections described on the following pages. Each section is to be numbered and labeled appropriately (i.e. Section 1 – Information Page, Section 2 – Service to Church and Parish …) For each section, identify activities or characteristics the nominee holds beyond that of an average council member. Nominees must be a member a) of the Knights of Columbus and b) of a Council in the Joliet Diocese Each council may submit only one entry per year from their own Council. Previously submitted nominees, may be re-submitted only by using this form (with all 8 sections). Previous Recipients are not eligible. Nominations for older members who were very active members in the past and are less active now should be submitted for consideration. Nominations for deceased members will not be considered. Nominations containing inaccurate or misleading information may be eliminated from consideration by the selection committee. Nominations can be submitted by postal mail or by email (Section 1 identifies where to send the nominations). All nominations must be received by February 20, 2015

Section 3 - Service to the Knights of Columbus (In this section, you can identify the offices held by the nominee in their Council, Assembly, Chapter or the State. You should also identify any activities or services the nominee chaired or was active in such as Committee work, Council development and reactivation, Degree work or Members they sponsored.)

Accomplishments can be Knight of the Month/Year, Family of the Month/Year, Senior Citizen of the Month/Year, Sir Knight of the Year, Master Medallion Award, State Knight of the Year, State Family of the Year and State Deputy Medallion. Also, you can include any Military or Community Awards the nominee has received.)
