Checklist for SMN Book Recommendations*
Purpose of this form: Before NPAs suggest a new book to campus, use this form to see if it meets these criteria. If it does, see “Submission Process” at the bottom of the form.
We recommend that an NPA together with the regional project director or regional nutrition and health education specialist complete this checklist. See submission process below.

If you have developed an alternative activity to go along with this book, please use the alternative activity template at http://extension.missouri.edu/fnep/smnalternates.htm and submit an alternative activity along with this checklist.
Submission process: 1. Complete the above checklist. 2. Submit checklist and a copy of the book to Di Hirt and Chris Jennings at 1205 University Avenue, Suite 300, Columbia, MO 65211 3. Di and Chris will review your submission within 4 weeks and provide comments. Your book will be returned to you.
