DUR Alert Definitions

Incorrect Drug Prescribed dose is less than or exceeds the usual adult or pediatric daily Dosage dose for target drug’s* common indications. No geriatric doses (age 65 and over) are included in the correct dosage warning screen at the present time.

Drug-Age Target drug is contraindicated in pediatric or geriatric patients. Conflict

Incorrect Target drug days supply exceeds usual maximum duration of therapy. This alert is Duration currently inactive for all drugs, including target drugs.

Drug-Disease Patient has known medical condition that contraindicates use of target Conflict drug.

Drug-Drug Drug-drug interactions create the potential for, or occurrence of, an Interaction adverse medical effect as a result of the patient using two or more drugs together. Drug-Drug Interactions are established by First Databank. Alerts are generated for all drug-drug interactions that are flagged as Severity Level 1, which is defined as, “drug combinations that are contraindicated and generally should not be dispensed or administered to the same patient.” This alert is currently active for all drugs, including target drugs.

Drug-Pregnancy Target drug is contraindicated or not recommended during pregnancy. Conflict

Drug-Gender Target drug is contraindicated in males or females. This alert is Conflict currently inactive for all drugs, including target drugs.

Therapeutic Target drug duplicates a drug in the same therapeutic or Duplication pharmacologic class for which recipient has an active prescription.

Drug-Allergy Patient has demonstrated hypersensitivity to target drug or drug Conflict class.

Overutilization Refill request interval for target drugs is less than 75 percent of (Early Refill) previous prescription’s day’s supply. This alert is currently active for all drugs, including target drugs.

Ingredient Target drug is an exact chemical duplicate of active ingredient in Duplication the patient's paid claims active medication history.

Underutilization Refill request interval for maintenance-use target drugs exceeds (Late Refill) 125 percent of previous prescription's day’s supply.

Clinical Misuse/ Target drug exceeds the threshold of three or more active Additive Toxicity prescriptions for narcotic analgesics, sedative hypnotics, certain antidepressants and/or antipsychotics.

* “Target drug” refers to the prospective DUR target drug list in Section 20 of this manual.
