TITLE OF POSITION: Dentist REPORTS TO: Dental Director DATE: January 12, 2005 STATUS: Exempt

POSITION SUMMARY:

As a member of the Provider team, this position is responsible for providing dental care to the patients of HCHN. This position will provide dental care for 95% of the time, and administrative responsibilities 5% of the time.

This employee must understand and support the vision and mission of HCHN. Understands that employment is contingent upon adherence to HCHN policies and procedures, JCAHO policies and procedures, OSHA standards, and other safety and health related codes and laws.

POSITION REQUIREMENTS:

Licensed to practice in Michigan preferably with dental family practice experience. Must have DEA registration. Must have a minimum of three (3) years of progressively responsible management experience in dental care related organization. Comprehensive knowledge of all phases of health care delivery, as well as the theory and practice of organizational management, preferably in a health care and/or not-for-profit environment. Must be able to communicate effectively orally and in writing.

PHYSICAL REQUIREMENTS/DEMANDS:

Requires vision and hearing of normal range. Must be able to bend, stretch or lift items up to 25 pounds. May be required to stand or sit for long periods of time. May be exposed to communicable diseases.

DUTIES AND RESPONSIBILITIES

1. Provides dental treatment to HCHN patients as evidenced by compliance with the following standards:

 Performs dental examinations, procedures, and prescribes medications to patients as assigned or as needed.*

 Advises patient on dental care education issues when needed.*

1  Effectively and efficiently completes all paperwork requirements for billing and medical records compliance.*

 Protects patients’ oral health by diagnosing and treating diseases, injuries, abnormalities of teeth and gums, and undertaking preventive procedures.*

 Advised patients on good health practices.*

 Ensures patients receive educational materials that pertain to their patients dental health needs.*

 Refers patients to specialists when required and in accordance with their insurance restrictions.*

 Performs procedures on patients in need. Develops and implements goals and objectives to ensure high quality, cost effective clinical care.*

 Ensures all patient charts are completed within 24 hours of the patient visit, and the charting is comprehensive and legible.*

 Plans and implements policies, procedures, and systems to achieve stated goals and to ensure compliance with JCAHO and other regulatory/third party mandates.*

 Regularly advises the management team on issues regarding operations and patient care to include staffing, data collection and interpretation clinic flow, appointment systems, client recall process, ancillary services and customer satisfaction.*

 Participates in the effectiveness review of the clinical operation activities which includes but is not limited to, patient flow, x-ray services, pharmacy services, referrals and all other activities within the clinics.*

 Implements procedures and clinical programming that would improve the continuum of patient care in an ambulatory setting.*

 Active participant in HCHN’s Performance Improvement Process, Provider’s meeting, Continuous Quality Improvement committee and EMG meetings as scheduled.*
