Application Form

The Melton Fund Mini-Grants are intended to allow teachers with a close association with UT-Dallas and/or its Science/Mathematics Education Program to enhance their delivery of STEM material in the classroom in many ways, including innovative approaches, more effective instruction, and access to non- textbook instruction (labs, demonstrations, etc.)

Typical Mini-Grants are limited to $100. This form is the complete application. It is straightforward to complete, but at the same time, it provides guidance that should be useful in writing proposals to other sources for larger grants.

You may type into this document or create a new document. If you type into this document, you should present your text as blue. The completed application is to be sent as an email attachment (in docx, rtf, or pdf formats only) to both the following email addresses: [email protected] and [email protected]. A. Basic Information

1. Name, mailing address, email address, and phone number(s) (including best times to call)

2. Educational Environment including school district, school, course assignments (course title(s) and grade level(s), and other assignments. You may add additional description of your educational environment (maximum 50 words)

3. Association with UT-Dallas Science/Mathematics Education. Typical categories include MAT student, MAT graduate, UTeach Dallas graduate, UTeach Dallas Mentor Teacher, UTD TRC Mentor Teacher. Please note all categories that are appropriate. If you do not fall into any of the listed categories, describe your close relationship to UT-Dallas and/or its Science/Mathematics Education Program.

B. Grant Request

1. Descriptive Title of Grant Request (maximum 25 words)

2. Amount Requested ($100 maximum)

3. What will be bought with grant funds? Note: In order to comply with UTD rules, the items will be purchased through UTD, and you will be notified when they are available for you to pick up or arrange for pickup. Provide sufficient detail for purchasing including supplier and supplier location/contact information, item number(s) and quantity, unit price(s), etc. UTD purchases are not subject to Texas sales tax.

1 C. Educational Payoff

1. In a statement of 100-300 words, describe how this Mini-Grant will help to connect your overall educational goals to daily classroom instruction/activities, and finally to improved learning in your classroom.

Note: This is your chance to sell your ideas. Use succinct, effective arguments to convince the people who will judge your application that you have a very good way to spend Mini-Grant money. The judges may be faculty in the Science/Mathematics Education Program as well as others who do not know educational jargon/buzzwords. Make your arguments effective to all the judges.

2. A final report is due by June 30 of the school year in which you receive the Mini- Grant. Instructions for the report: In a statement of 100-300 words, describe how this Mini-Grant helped to connect your overall educational goals to daily classroom instruction/activities, and finally to improved learning in your classroom. Be sure to include personal learning experiences, often of the form “Next time, I will know better, and I will do it this way.”

Note: You will not receive a subsequent Melton Fund Mini-Grant unless the final report has been received and approved.
