To complete this scoring matrix: 1. Identify your facility requirements. 2. Assign each requirement a priority value. For this example, Connie assigned the requirements that she felt the facility must have a value of “10” and those that she would like to have but can do without a value of “5.” 3. During the facility visit, assign each location a raw score for each requirement. In this example, Connie used a scale of 1-5 for the raw score, where “1” was the lowest score and “5” was the highest score. 4. Multiple the priority value and the raw score for each requirement for each facility. Record the product in the total column. 5. Add up all the totals for each facility to get a final total.
