New Student Guide for Initiating Services (Online Intake)

Step 1: Click or Insert UNCW DRC Online Student Portal Link into an internet browser. You will be linked to the DRC Online Portal main page (see Example 1A). Click “Online Intake” and you will be directed to the “Registration Form.”

Example 1A: Step 2: Please complete the entire registration form by inputting the requested information into the appropriate fields.

 Documentation Upload

o Question 5 allows you to upload medical documentation (IEP/504 plan, Psychological Evaluations, Doctor’s Note, etc.) into our confidential and secured system. This information is forwarded directly to the Disability Resource Center Staff.

 Release of Information

o At times it may be necessary for Disability Resource Center staff to obtain additional information from specific individuals. This information is used solely for determining appropriate accommodations and supporting you throughout your academic career at UNCW.

 On-Campus Housing Accommodations

o Request forms for an accommodation in on-campus housing can be found on the DRC website.
