As the autumn season draws near, we are excited to invite you to the

Iowa Department for the Blind

Fall 2015 Transition Retreat!

WHEN: Friday, October 16 - Sunday, October 18

WHERE: Sunstream Retreat Center 1130 Juneberry Road Ogden, Iowa (515-275-3078)

Sunstream Retreat Center is nestled in a beautiful rural Iowa setting and features many opportunities for fun and socializing including team building and recreational activities such as zip-lining, hiking, camp fires and wagon rides.

WHAT: The focus of the retreat is on job readiness, particularly in the areas of interviewing, job shadowing, and networking. Retreat participants will have opportunities to learn about how to conduct an informational interview, what to do on a job shadow, and why and how to network. These skills are critical in the career prep, exploration process.

WHO CAN PARTICIPATE? Students who are: 1. At least age 14 and have completed 8th grade 2. Clients of the Iowa Department for the Blind

HOW TO PARTICIPATE:

REGISTRATION: The registration deadline is September 24!

1. Complete the attached registration form and email it to [email protected]

2. Please print out, complete and sign the forms requiring signatures and mail to the Iowa Department for the Blind; 524 4th St.; Des Moines, IA 50309 OR provide them when you arrive at the retreat.

PLEASE NOTE: 1. If the retreat participant is a minor or is not his/her own legal guardian, a parent / legal guardian must sign these forms. If the retreat participant is 18 and is his/her own legal guardian, he/she must sign the forms.

2. If you prefer a hard copy of the registration packet mailed to you, please contact us to request one.

REQUIRED ITEMS TO BRING TO THE RETREAT: 1. Materials /supplies to take notes 2. Toiletries - shampoo, soap, brush, toothbrush, deodorant, etc. 3. Backpack 4. Medications 5. Sunscreen 6. Casual clothing including: a. A set of clothing and shoes appropriate for outdoor activities. b. Outerwear based on the weather (jackets, sweatshirts, coats, hats, gloves, boots, etc.)

NOTE: Bedding and bath towels WILL be provided.

ARRIVAL AND PICK-UP TIMES: Participants are to arrive at Sunstream by 5:00 PM on Friday, October 16 and are to be picked up at 12:30 PM on Sunday, October 18.

DIRECTIONS TO SUNSTREAM RETREAT CENTER: (FYI -- GPS systems may not be accurate) From US-30 W: 1. Turn right on J Avenue & drive north for 1.5 miles. 2. Turn right onto 208th St & drive 3/4 of a mile. 3. Turn right onto Juneberry Rd & drive 1/2 of a mile. 4. Turn right into Sunstream
