This clinician check is to be carried out following the first pharmacy check. For clinical trials the clinician checker should be PI for the trial in one TVCN institution. Please prescribe the regimen in institution TVCN for the dummy patient below. You may have to discontinue the previous regimen for that patient in order to do so. Do not error. Detail any changes required overleaf, and send to the network pharmacists. The changes requested will then be made and a second pharmacy check will be carried out and then approved for use on Aria.

PLEASE ALLOW 2 WEEKS FOR THE SECOND PHARMACY CHECK AND TAKE THIS INTO CONSIDERATION BEFORE BOOKING TREATMENT FOR PATIENTS.

Information Check that the ‘overview’ and ‘chemo order instructions’ are on the Start consistent with TVCN chemotherapy tumour site protocol, or Treatment trial protocol. Check overview includes all drugs, doses, page. route, cycle frequency& no of cycles. Check anti-emetic is as per TVCN policy 3. Drugs Having ordered the regimen, check all drugs, doses, route, and admin instructions are correct as per TVCN protocol. 4. Dose Adjust Dose button. Check that all drugs can be modified as modification you would expect and the dose banding /rounding is working. % reductions are correct. Click on the double red arrow and check banding tables are the same as the TVCN banding table. 5. Support Check that the appropriate support regimens are available regimen under Favourites, Support tab. 6. Non cyclical Having approved cycle 1, go to future cycles on the drugs Treatment tab, check that non cyclical drugs change as you expect. (eg McAb dose drops cycle 2) 7. Cycles Check that the default number of cycles and frequency is appropriate in the first instance. 8. Toxicity Check toxicity recording page has appropriate toxicity ranges for the regimen set as default. 9. Scheduling Close the patient, go to schedule, Check target appointment dates fit with regimen appointments you would expect. Eg Day 1 and 8.
