Children Served with Missing Clinical Diagnosis

Report Background The Children Served with Missing Clinical Diagnosis report can help Local District and Voluntary Agency supervisors and managers monitor children without a clinical diagnosis and improve the clinical diagnosis process. Report Purpose Provides Local Districts and Voluntary Agencies with a detailed list of children served during a particular AFCARS submission who have a missing clinical diagnosis in CONNECTIONS. The report includes Case, Stage, and Health Responsibility information for each child. Target Users Managers and supervisors from Local Districts and Voluntary Agencies. Classification Operational report Prompts Upon opening the report, users are asked to select one or more values from a list of choices for the following report prompts:  District/Agency: Select the name of the agency (Local District / Voluntary Agency) for which you want to run the report. Agencies are listed alphabetically by agency code. Notes LDSS version only includes cases where the Case Manager is from the District selected at the prompt; VA version only includes cases where the Case Planner is from the Voluntary Agency selected at the prompt. Data is refreshed monthly. If CONNECTIONS data is missing, it is either because the child's CIN is not in CONNECTIONS or because the child is not being tracked in a CONNECTIONS Family Services Stage. The “children served” population includes children who were (1) in care during the 6-month reporting period; or (2) discharged during the 6-month reporting period; or (3) on trial discharge for more than 182 days during the 6-month reporting period. A “missing clinical diagnosis” includes children who are missing (1) an AFCARS disability diagnoses in any health Domain Type; or (2) a non-AFCARS disability diagnoses in the Physical/Medical health Domain Type with an Appointment Type of Initial Assessment, Reassessment, or Well Child; or (3) any diagnosis in the Physical/Medical health Domain Type with an Appointment Type of Initial Assessment, Reassessment, or Well Child.

Data Item Data Definition

LDSS/Agency Name The name and 3-digit agency code for the Local District or Voluntary Agency selected at the report prompt. (ACS is listed alphabetically as “New York City DSS”.) Since this report provides detailed information, Local District staff can only run this report for their own District. Voluntary Agency staff can only run this report for their own Agency. State staff can run this report for more than one District at a time.

Total Number of A count of the CIN IDs for the report. Children Served with Missing Clinical Diagnosis Sort by Select the data item by which to sort the report. LDSS version: The default sort order is by Case Manager. Choices include Case Manager, Case Planner Agency, Case Planner, CCRS Case ID, Child CIN, Child Name, and Health Responsibility Agency. VA version: The default sort order is by Case Planner. Choices include Case Planner, Case Manager Agency, Case Manager, CCRS Case ID, Child CIN, Child Name, and Health Responsibility Agency. Report Date The date on which the report was run. Data as of Date The last time data was extracted from CCRS and CONNECTIONS for this report. Data for this report is refreshed once a month. AFCARS Reporting The six-month period for which the data is submitted to AFCARS. This Period will be either Apr - Sept or Oct - Mar. Case Manager The full name of the Case Manager assigned to the Stage in CONNECTIONS. Case Manager The Office of the Case Manager assigned to the Stage in Office CONNECTIONS. (LDSS version only) Case Manager Unit The Unit of the Case Manager assigned to the Stage in (LDSS version only) CONNECTIONS. Case Planner The 3-digit agency code and agency name associated with the Case Agency Planner. (LDSS version only) Case Planner The full name of the Case Planner assigned to the Stage in CONNECTIONS. Case Planner Office The Office of the Case Planner assigned to the Stage in (VA version only) CONNECTIONS. Case Planner Unit The Unit of the Case Planner assigned to the Stage in (VA version only) CONNECTIONS. Case Manager The 3-digit agency code and agency name associated with the Case Agency Manager. (VA version only) CCRS Case Id The Case ID associated with the child. CONNX Case Id A unique number assigned to child welfare cases by CONNECTIONS. Case Name The family name by which a child welfare case is identified. Child CIN Child Client Identification Number; a unique identifier for the child that is generated from the CCRS application. Child Id A unique identifier for the child that is generated from the CONNECTIONS application. Child Name The first name, middle initial, and last name of the child. Included are children who were (1) in care during the 6-month reporting period; or (2) discharged during the 6-month reporting period; or (3) on trial discharge for more than 182 days during the 6-month reporting period AND who are missing (1) an AFCARS disability diagnoses in any health Domain Type; or (2) a non-AFCARS disability diagnoses in the Physical/Medical health Domain Type with an Appointment Type of Initial Assessment, Reassessment, or Well Child; or (3) any diagnosis in the Physical/Medical health Domain Type with an Appointment Type of Initial Assessment, Reassessment, or Well Child. Stage Id The Family Services Stage ID associated with the tracked child. It is important to note that a tracked child may have been in more than one open Stage during the Report Period. Stage Start Date The date the Family Services Stage was opened in CONNECTIONS. Stage End Date The date the Family Services Stage was closed in CONNECTIONS. Children in closed Stages may appear in this report if they meet the “children served” and “missing clinical diagnosis” criteria for the AFCARS Reporting Period. Health The name of the agency responsible for maintaining the health Responsibility information for a particular child. If the field is blank, no agency has Agency been assigned the “Designate Health Responsibility” role in CONNECTIONS.
