Please complete this form for employees who are leaving the company, checking off each item as it is completed. Please sign and date when complete and return form to HR.

______2. Ensure the employee has reviewed and approved final timesheet on the timekeeping system. Employee is informed that final paycheck will be direct deposit if the employee currently receives direct deposit or a live paycheck if the employee currently receives live paychecks. Employee is also informed that 1) final paycheck will be prepared on the next regular payroll run 2) final paycheck will be mailed to employee unless they specifically make other arrangements in advance and 3) Final commissions will be paid to exiting employee when commissions are scheduled to be paid to the rest of NPV employees.

______3. If employee is moving, list new address and phone number below (or address and phone number of a contact). Be sure to transfer this information to the Record of Termination Form. The employee is advised that they must keep Human Resources informed of any future changes for Benefits and W2 tax information.

______7. After confirming that HR has received all of the items listed under number 6, shred and discard all items.
