2016 /2017 IREDELL COUNTY BENEFITS SUMMARY

Health Insurance  Provided by CIGNA.  Effective first day of month following 30 days of employment.  Choice of OAP (PPO) plan or High Deductible plan with Health Savings Account.  Spouses can only be covered under the County Health Insurance Plan if they have no Health Insurance coverage available through their own employer.  Rates for either plan choice (7/1/16 – 6/30/17): o Individual – no cost to employee with wellness component; $60/month without Wellness component. o Employee/Child - $254/month with Wellness component; $314/month without Wellness component. o Employee/Children - $364/month with Wellness component; $4247/month without Wellness component. o Employee/Spouse - $287/month with Wellness component; $347/month without Wellness component. o Family - $469/month with Wellness component; $529/month without Wellness component.

Dental Insurance  Provided by Ameritas Life Insurance Company.  Effective first day of month following 30 days of employment.  Rates (7/1/16 – 6/30/17): o Employee Only - $33.14/month o Employee/Child or Children - $80.78/month o Employee/Spouse - $66.28/month o Family - $106.02/month

Vision Insurance  Provided by Community Eye Care.  Effective first day of month following 30 days of employment.  Choice of Comprehensive Plan and Eyewear Only Plan.  Comprehensive Plan Rates (7/1/16 – 6/30/17) o Employee Only – $9.08/month o Employee/Child(ren) - $16.48/month o Employee/Spouse - $19.24/month o Family - $26.64/month  Eyewear Only Plan Rates (7/1/16 – 6/30/17) o Employee Only – $5.18/month o Employee/Child(ren) - $8.58/month o Employee/Spouse - $9.98/month o Family - $13.38/month

Retirement  NC Local Govt. Retirement System (Mandatory) - 6% required contribution made by employee  Prudential 401(k) Plan – 5% contribution made by County; if desired, employee may contribute an additional amount.

Life Insurance Coverage  Life Insurance equal to 1 times annual salary up to $75,000 provided by the County  Optional dependent, term and whole life insurance policies available through payroll deduction; rates vary w/ level of coverage selected.

Other Insurance Coverage Available  Cancer insurance – rates vary w/ level of coverage selected.  Disability insurance– rates vary w/ level of coverage selected.  Critical Illness Insurance- rates vary w/ level of coverage selected.  Personal Accident Insurance– rates vary w/ level of coverage selected.

Miscellaneous  Section 125 Cafeteria Plan w/ health care and child care reimbursement accounts.  Employee Assistance Program.  Employee Health Clinic

Leaves  Annual Leave - 10, 12, 15, 18, 21, or 24 days annual leave per year depending on length of service.  Sick Leave - 12 days sick leave per year.  Paid Holidays - 11 or 12 holidays.  FMLA – Employees are eligible for FMLA after employed at least 12 months and have worked at least 1,250 hrs in previous 12 month period.  Child Involvement Leave – Employees granted up to 8 hrs of unpaid leave for parental involvement in schools, to meet with a teacher or administrator of any school; and tutoring and mentoring in the schools with a community service organization.  Military Leave – An employee who is a member of the National Guard or Armed Forces Reserved is allowed 10 workdays of military training leave annually with partial compensation.  Civil Leave – An Employee called for Jury duty or whose official job duties require attendance in court or at a job related legal proceeding will be considered work time and may use civil leave with pay for the period of time required to be at court and will receive any fees received for jury duty.  Educational Leave – An employee meeting the eligible criteria for educational leave may request a leave of absence at full-pay for one course at a time or receive reimbursement for (1) course at a time taken outside of working hours which will better equip the employee to perform assigned duties.  Worker’s Compensation Leave – Provides coverage to all full and part-time employees to cover medical expenses and lost time from work due to work-related injuries.  Administrative Leave – Administrative Leave with pay may be granted by the County Manager when circumstances are such that the manager deems it to be in the best interest of the county.  Shared Leave – This provides an opportunity for employees to assist other employees affected by qualifying FMLA condition that requires an absence from work for a prolonged period of time (a minimum of 160 hrs) resulting in possible loss of income due to lack of accumulated paid leave.

Wellness  For the benefit plan year beginning 7/1/16, employees who meet one of the following benchmarks will be eligible for the wellness discount on their health insurance premiums. *Meeting 4 out of 5 biometric criteria regarding blood pressure, waist circumference, cholesterol ratio, fasting blood glucose, and tobacco usage. Employees are also required to complete an online Health Risk Assessment. *For employees meeting fewer than 4 of the benchmarks, they may still receive the wellness discount by meeting with the wellness nurse once per quarter (for two quarters they may opt to participate in a wellness activity in lieu of the nurse visit) *Employees hired 7/1/16 or after will be required to comply with the requirements upon hire in order to be eligible for the discounted health insurance premium rate.  All spouses and dependents (over the age of 18) covered under the Health Insurance Plan are required to participate in the Wellness Program in order to receive a discounted rate on Health Insurance.

 Gym Membership Iredell County Partners with local gyms in order to provide reduced membership dues for employees. Discounted membership rates are available for the City of Statesville Fitness Center, 24/7 Total Fitness, and Rock Fitness.
