3 Review of Draft Survey The purpose of the survey is to capture basic information about the sites that will best aid us in designing the programming, but also will serve as a resource to each of you in collaborating with one another.

A number of recommendations were made to the survey. Heather will incorporate these and send out the link to the Survey Monkey survey. 4 Patient Communications The PCPCC has put forth a number of educational aids for practices in communicating the About the Medical concept of the Medical Home. People felt that it would be very worthwhile to work together on a care plan template that could be automated (even if sites then took the template and modified it to best meet their needs). People also felt that it would be worthwhile to work together on patient engagement including materials, methods, messaging and measuring experience of care.

We will work on an extended agenda for 6/23 at a bigger location (with food) to facilitate the
