POST: LOCUM CONSULTANT IN GENERAL ADULT PSYCHIATRY

BASE: ARRAN CENTRE/PARKHEAD HOSPITAL

Applications are invited for a locum full-time post in General Adult Psychiatry in East East Glasgow (The Arran Resource Centre), which is part of Northeast Glasgow Division of psychiatry. Mental Health services in East Glasgow are provided by integrated health and social work multidisciplinary CMHTs, supported by a Crisis Team and a Primary Care Mental Health Team.

The post is based at Parkhead Hospital and the Arran Centre Community Mental Health Team (CMHT), and comprises of 10 PAs allocated between Parkhead Hospital and Arran Centre CMHT. On call responsibilities will be shared with other consultant colleagues on an approximately 1 in 28 rota.

The postholder will be supported by appropriate medical secretary time, and by junior doctor input for inpatient sessions. Responsibilities will be shared between a total of 19 general adult psychiatrists in Northeast Glasgow, and the successful applicant would be expected to work closely with colleagues to provide a full range of services to inpatients and outpatients. Part of the job plan usually involves covering group task responsibilities- usually equivalent on one programmed activity.

The successful candidate would be encouraged to take the opportunities available for further education and continuing professional development. There are active education and audit programmes within the service.

Job Description JOB DESCRIPTION

This locum full-time post in adult general psychiatry in the city centre sector of East Glasgow. For a fixed term of four months

Current Psychiatric Services – Northeast Glasgow Area

The psychiatric services in Glasgow are split into three areas of a city wide mental health partnership (MMP); two areas North of the River Clyde and one to the South. The Northeast Glasgow area lies between the River Clyde to the South, the River Kelvin to the West and the Campsie Hills to the North. It covers a population of approximately 350,000 with areas of significant deprivation. Community Services in Northeast Glasgow are well developed. The general adult services are provided by CMHT’s based in 6 community resource centres:

Arran Centre / City Centre Locality (G1, G2, G3, G4, G21, G31, G40) population approx 50,000

Anvil Centre / Mid Sector Locality (G32, G33) population approx 50,000

Auchinlea House / East Sector Locality (G33, G34, G69, G71) approx 50,000

Larkfield Centre / Strathkelvin Locality (G33, G65, G66, G69), population 85,000.

Springpark Centre / Springburn/Possil Locality (G21, G22, G33), population 50,000

Shawpark Centre / Maryhill Locality (G3, G4, G20, G23), population 53,000

The Acute Adult Inpatient Beds are presently located on 2 sites: 1) MacKinnon House on the Stobhill General Hospital site and 2) Parkhead Hospital in East Glasgow.

1) Parkhead Hospital Site Parkhead Hospital opened in 1988 and comprises 5 wards, 3 adult admissions and 2 elderly admission wards situated adjacent to the Anvil Centre. Occupational therapy and physiotherapy/gym are provided on site. There is a medical library and on-call accommodation for SHOs. The Mental Health Tribunal holds hearings in the Millan Suite at Parkhead Hospital. IPCU facilities, Addiction beds and ECT are based at Stobhill. East Psychotherapy and psychology are based at Carswell House.

Greater Glasgow Health Board’s “Modernising Mental Health Services” strategy has resulted in major investment in new and innovative mental health services, including early intervention and perinatal mental health services. The perinatal mental health services have opened in the Southern General Hospital to the south of the city.. A primary care mental health strategy has been established to improve the management of patients whose needs are best met within primary care rather than secondary services. There are around 13 junior medical trainees on the East Glasgow Training Scheme. Specialist Registrars are attached to SpR trainers

There are 13 Trainee’s on the North Glasgow Training Scheme.

1. Clinical Services

The city centre catchment area includes several of the most deprived localities in Glasgow, along with an area of affluence in the Merchant City. Three consultants already work in the city centre - two in general adult psychiatry and the other, Dr Moore, working in continuing care for people with severe and enduring mental health problems using an assertive outreach approach. Dr Moore also has 8 rehabilitation beds for the northeast area based off hospital site and no acute in-patient commitments.

At the present time the psychiatric services in Glasgow are subject to considerable change with configuration of services around primary care practices and the establishment of Community Health Partnerships. The inpatient services in the northeast sector are also in a process of regeneration with a gradual move from the Parkhead site to Stobhill already underway. The catchment areas for the Arran Centre and adjacent resource centres in North and West Glasgow are likely to change substantially in the coming years. It is therefore likely that the consultants at the Arran Centre, including the successful applicant, will find their jobs subject to a similar evolution reflecting the needs of the service.

Much of the service is delivered by the team in or close to the patient’s home. However out-patient clinics take place within the centre. There are good links with primary care and with the many voluntary and statutory providers within the locality. There is also a large inpatient multi-disciplinary Team. The in-patient service has recently benefited from additional funding to improve the nursing staff/patient ratio and improve the therapeutic activities on the ward. There is close working between members of the respective teams.

The post holder will be expected to provide additional medical leadership to the teams and assist with the further development of the service.

2. Supporting Staff

The community multidisciplinary team are based at the Arran Resource Centre, an excellent and spacious building, which opened in November 2003. The team comprises community psychiatric nursing, occupational therapy, psychology and social work with input from pharmacy and supported by the Locality Manager.

The inpatient multi-disciplinary team are presently based at Parkhead Hospital and comprise nursing, lifeskills, pharmacy, with input from community-based psychology, social work, nursing and occupational therapy.

The post-holder will have Specialty Doctor Grade support or alternatively may supervise an attached trainee from the East Rotation Training Scheme.

3. Duties of the Post

The post holder will have responsibility for providing a comprehensive range of services in conjunction with the multi-disciplinary team to patients with severe and enduring mental health problems within the locality between the ages of 16 to 65. He or she will supervise in-patients at Parkhead Hospital and do out-patient and CMHT work at the Arran Centre including Staff Grade and designated nursing staff support.. He or she will be expected to cover for colleagues during absences in line with the arrangements set out in the National Terms and Conditions.

The postholder will, with the agreement of the clinical director and divisional medical director agree a job plan. The final timetable is flexible, subject to the needs of the service and would be discussed with the successful candidate. An example of programmed activities (4 hours) commitment would be: Direct Clinical Care 9 programmed activities (includes 6.5 programmed activities emergency, on-call, in-patient and community work and including 1 programmed activity of an area group task.) Group task duties are nominal 4 hours per week of work. Current examples in the area are clinical tutor, RMO for a long stay ward, ECT lead, lead clinician, undergraduate tutor etc. Initially the group task 4 hours would probably be as RMO for one of the 3 Orchard wards at the old Ruchill Hospital site. These wards contain rehabilitation and continuing care patients. This may change subject to negotiation and service need.

Supporting Professional Activities 1 programmed activities

The post holder must complete section 22 training of the Mental Health (Care and Treatment) (Scotland) Act to allow approval by the NHS GG and C Board as an Approved Medical Practitioner ( AMP). This training should have occurred prior to taking up post or as soon as possible thereafter. Being an active AMP is a core requirement of the job and Mental health legislation work is incorporated in the 10 programmed activities job plan.

The on-call commitment is currently 1 in 28, covering the North East. The Trainee’s at MacKinnon House and Parkhead Hospital work on New Deal compliant partial shifts. There is Trainee middle grade cover to the North East on most nights and weekends.

4. Teaching and Research The service regularly receives senior medical students from the University of Glasgow on their Psychological Medicine attachments. The post holder will be expected to become actively involved in undergraduate teaching. Each Consultant can expect up to 5 attachments per year, of one student at a time, for 5 weeks.

In recognition of the teaching commitment the University of Glasgow may grant to the post holder the status of Honorary Clinical Senior Lecturer in Psychological Medicine. In addition to the educational supervision of the attached trainee, the post holder will be expected to contribute to the Post-Graduate Teaching programme of journal clubs, case presentations etc. at the East/Internal Teaching sessions and may also be asked to contribute to the Glasgow Wide/External Teaching Programme. Internal teaching alternates with External teaching on Thursday mornings during term time.

Multidisciplinary Teaching There is opportunity for multidisciplinary teaching within the service from time to time and this is encouraged as a contribution to team / staff development.

Research and Audit There are a number of research projects on going in Northeast Glasgow and the post holder will be encouraged to undertake/become involved in research where appropriate. The post holder would be expected to participate in audit. 5. Continued Professional Development

Study Leave There is a generous study leave budget and medical staff are encouraged to ensure that they keep their practice up to date.

CPD There is a West of Scotland CPD Programme, with speakers invited to talk on clinical or research topics, organised by the University of Glasgow. Consultants’ appraisal systems are in place and an established peer review system has been running for a number of years.

6. Management/Administrative work

The Mental Health Division has recently undergone a management restructure primarily to support clinical governance and to focus on clinical service delivery. This has created a city wide Mental Health Partnership overarching the 5 city CHPs including the East CHP, the principle CHP for this locality. The current Area Management Team would encourage the post holder to be active in clinical input to managerial planning.

The point of contact between managers and clinicians is at the monthly clinical business meeting where current clinical service issues are discussed.The post-holder would be a member of the East Division of Psychiatry. The North and East Divisions may merge following further consultant discussion. 7. Date when post is vacant.

GRADE AND SALARY Consultant £ 74,504 £ 100,446 per annum (pro rata)

New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.

HOURS OF DUTY Full Time 40.00

SUPERANNUATION You have the option to join the NHS Superannuation Scheme, to participate in the State Earnings Related Pension Scheme or to take out a Personal Pension. Employee’s contributions to the NHS Scheme are Tiered based on your earnings and the employers contribution equates to 13.5 % of salary. Employees in the NHS Scheme are “Contracted-out” of the State Earnings Related Pension Scheme and pay a lower rate of National Insurance contributions. Employees who choose to participate in the State Earnings Related Pension Scheme pay the higher rate of National Insurance contribution. A Stakeholder Pension is also available. A Personal Pension is a private arrangement agreed with the pension provider that will be an organisation such as a Bank, Building Society or Insurance Company.

REMOVAL EXPENSES Assistance with removal and associated expenses may be given and would be discussed and agreed prior to appointment.

This post is considered to be in the category of “Regulated Work” and DISCLOSURE SCOTLAND therefore requires a Disclosure Scotland Protection of Vulnerable Groups Scheme (PVG) Membership which currently costs £59.00.
