Title: Lifestyle Coach Hourly rate: £10.00 Responsible to: Health and Wellbeing Manager Location: YMCABC Temple Street and a number of outreach locations in and around the Black Country.

Background : YMCA Black Country Group is a group of YMCA charitable companies & associated trading subsidiaries operating in the Black Country and South Staffordshire area. The charity employs around 230 staff across several sites, delivering a wide range of community services including supported housing, youth work, childcare and family support, drug and alcohol support interventions, skills and employment training, health and wellbeing services, and work in prisons.

Job Role: To support the Changing UR Health 4 Life team to deliver health and wellbeing activities to communities in the Black Country. You will provide the motivation and encouragement to clients to enable them to achieve their health and wellbeing goals, and to reach their full potential. You will deliver and plan one-to- one’s and group activities. Accurate record keeping, impact assessment and data entry are vital to the role to ensure the project meets funding requirements. You must have your own transport to enable you to travel to offsite locations around the Black Country. The post holder must be able to respect the Christian Ethos of the YMCA and uphold its values. A DBS clearance will also be required.

Duties and Responsibilities

1. To make contact with the identified client group through direct referrals, partner referrals and through pro – active working. 2. To complete lifestyle assessments with clients creating a personalised care plan of interventions with regards to health and wellbeing support programmes. 3. To undertake joint assessments with other team members and other agencies. 4. To agree with the client an action plan that takes into account their full range of needs with a specific focus on Wellbeing, upskilling and building the confidence of the clients. 5. To plan and organise health and wellbeing activities and programmes for individuals. 6. To be clear about and establish referral pathways as an internal service as well as to external partners in relation to health and wellbeing.

7. To utilise the central data system, ensuring that data and progress is recorded accurately and in a timely manner. 8. To produce reports on progress of clients and numbers engaged, ensuring this is inputted within a wider reporting framework for funding and destination tracking purposes. 9. To liaise with internal and external staff and agencies as appropriate. 10. To maintain records, financial data and statistics as directed by the Line Manager. 11. To be responsible for material assets supplied as part of the post (e.g. lap top, mobile phone, project equipment etc) 12. To create and deliver engaging lesson plans and workshops. 13. To deliver one-to-one lifestyle assessments and monitor and capture progress as outlined through the 24 week programme. 14. To arrange and/or run activities including healthy eating programmes, relaxation workshops at outreach locations across the Black Country. 15. To maintain good levels of communication and provide regular reports to the Health and Wellbeing Manager ensuring all outcome/targets are being achieved. 16. To ensure all aspects of policies and procedures are followed at all times with a positive attitude and a high level of consistent service. 17. To be client focused, polite, professional, engaging to clients and colleagues, creating a warm and welcoming atmosphere. 18. To support with the organising of sessional staff and volunteers maintaining good working relationships, ensuring the priorities of the roll are met. 19. To work within set budgets provided by your Line Manager and to be responsible for monthly reviews for each client. 20. Undertake other general duties as appropriate to the post.

Person Specification Job Title: Health and Wellbeing Coach Desirable Essential Experience  Delivering engaging Health and Wellbeing programmes Organising and delivering one-to-one lifestyle assessments & advice for people  within recovery. Delivering health and wellbeing sessions and activity programmes relevant to the  cohort. Developing engaging session plans on a weekly basis. 

Working independently on own initiative with little day-to-day supervision. 

Encouraging healthy lifestyle change within disadvantaged individuals & groups. 

Delivering consistent results based on set targets and objectives.  Understanding the requirements of quality standards to be achieved within the  health and wellbeing arena. Working with people with a complex list of needs and challenges, (e.g. Including  health and well-being, substance misuse issues and those at risk of offending behaviour.)

Qualifications  Willing to Life Coach train  Nutrition  Level 3 fitness

One of the following recognisable - Wellbeing Advisor/ Fitness coaching  qualification and/or 2 years relevant experience Alcohol awareness   Willing to Current first aid qualification train Adult teaching qualification/ IAG 

Skills  Demonstrable time management skills. Ability and confidence to deliver sessions to disadvantaged adults, and to be able  to maintain client commitment and involvement. Good budgeting skills – able to keep within budget for sessional staff and delivery  activities. Able to produce excellent reports on outcomes, progression and service  evaluations. Excellent interpersonal skills, which put clients, volunteers and peer mentors at  ease and maximises their potential development. Excellent administration and accurate record keeping ability providing information  as required to deadline. Good verbal and communication skills (1-2-1, group work) and ability to write  reports Ability to work under own initiative and to manage case load. 

An ability to work alongside partner agency staff. 

Knowledge  Demonstrate excellent knowledge of what constitutes a healthy lifestyle.

Demonstrate clear understanding of the nutritional benefit of a variety foods and  the application of this in everyday life.

Awareness of client’s budget constraints and the need to demonstrate how to  manage money. Understanding of drug and alcohol misuse and the ability to support clients on a  holistic level.
