Statewide Portable Equipment Registration Program FORM 24 - Portable Equipment Unit – Daily Recordkeeping Form (Auto-fill format. Use “Tab” or up/down arrows to enter information. Additional form information is provided on the reverse side.)

PERP Registration forms are available at http://www.arb.ca.gov/portable/portable.htm.

This form must be kept at a central place of business. It must be maintained for five years and made accessible to the Air Resources Board Executive Officer or air districts upon request.

1. Company Name - Legal name of entity, business, organization, agency or private individual that operates the portable equipment unit.

2. PERP Registration Number - Enter the registration number from the registration certificate or sticker.

3. Location - Enter the location using street address and city; county and UTM coordinates; or other location indicator where the equipment unit operated or was located on the date indicated. The location should be entered for daily operation and also each time the equipment unit is brought to a new location.

4. Date – Enter the date (month, day, and year) the equipment unit was at the location specified. The date should be entered for daily operation and also each time the equipment unit is brought to a new location.

5. Amount of Material Processed – Enter the quantity of material processed. Use the units as listed in the Operating Conditions for the equipment unit or the units listed below for the type of equipment.
