Multiple Grave Sites

Please use this form to register more than one grave (i.e. a Bahá’í plot with several graves)

Return the completed form to: National Spiritual Assembly of the Bahá’ís of the United Kingdom 27 Rutland Gate London SW7 1PD A separate set of forms is required for each location, so if for example there is more than one cemetery in your jurisdiction, please keep all the graves in each cemetery on separate forms. Notes for each of the items are shown on the accompanying form.

Please complete the form clearly and IN CAPITAL LETTERS.

1) Enter full name and title. Try and spell as accurately as possible, but do not delay because you are not sure, as we can usually find everyone.

2) Maiden name this will be useful for future research, but is not essential now.

3) Date of birth: format dd/mm/yyyy

4) Do not worry if you do not know the Bahá’í membership number, we can look this up.

5) Full date of death or closest approximation. We can work out the Bahá’í date as long as we know the time the person died.

6) Date of burial (only if known) – it will help to have the date the person was buried should further research be required.

7) Is there a headstone? For recent burials the headstone may take a year or more to be put in place.

8) Only if known – they may be buried outside of the community in which they lived.

9) Current owner of the plot – this may be the Local Spiritual Assembly or a relative, for example if the area registered Bahá’í too, please give their ID number.

10) The current owner’s address – if not the Local Spiritual Assembly. If this may be difficult to obtain leave bland or mark as ‘unknown’. 11) All graves have a plot number in addition to the headstone which is usually visible on the grave.

12) Most graves will expire after a certain number of years. This will vary with each authority. It is important to know this date so that a case for renewal may be put in good time.

We need as much information as possible, but anything is better than nothing at all. A diagram of the actual location of each grave may be added on a separate sheet.
