Rules for Submission of Application:

 Funds may only be requested when the applicant has been denied funds to attend a food safety related training or conference by his/her employer.  Applicant must submit a letter of denial of funds and approval for participation from their immediate supervisor along with this application.  A copy of the training or conference brochure will be submitted along with this application.  Scholarship applications received will be evaluated by the GLCFP executive board.  If the application is approved, an award notice will be issued to the applicant.  Scholarships up to $350 will be offered for travel/training in-state and scholarships up to $500 will be offered for travel out of state.  Applicant must submit an explanation of how attendance at the food safety training or conference will enhance their job performance, along with this application. (Use separate sheet if needed)

Note:

 Please allow at least two weeks for processing of this applicant form.  Membership in the Great Lakes Conference on Food Protection is not required; however members will receive priority status.  Following the training or conference applicants receiving scholarship awards must submit a letter to the GLCFP executive board stating what they learned at the conference and how it helped further his/her professional development.  Refer to back page for further information on how this Scholarship application process is conducted.

Applicability: This applies to funding of approved continuing education scholarships by the Great Lakes Conference on Food Protection.

Approval: The application must have been approved by the GLCFP Board prior to the conference/training.

Process:

1. It is recommended that cost be minimized as much as feasible, particularly as it applies to travel. Search for lowest possible airfare with reasonable schedules. If traveling by car, carpool if possible.

2. All costs will be paid by the scholarship participant, and then reimbursed by GLCFP. Reimbursement will be paid directly to scholarship participant up to the maximum scholarship award amount upon providing receipts.

3. Mileage will be reimbursed at the current Federal rate.

4. After the travel is completed, the scholarship participant shall compile, by date, their reimbursable expenses, along with receipts. (Reimbursable expenses are defined as: Conference or Training registration, travel, and meals.) All submissions shall be received within 30 days after attendance of the event.

5. The scholarship participant shall also include, with reimbursement documentation, a brief paragraph identifying the food safety training/education or policy development benefit(s) accrued as a result of the authorized travel.

7. Employee shall retain a copy of all documentation for their records.
