This form is for undergraduate student use only. Students who have not been enrolled at Northern Arizona University for three consecutive terms (summer & winter terms excluded). Students who have not been out for more than three semesters, but have earned a bachelor’s degree and/or certificate from NAU should also complete this form. In order to begin a second bachelor’s program at NAU this form is needed. Note: If you were academically suspended from NAU, you must also submit a “Reinstatement Appeal Form” by the published deadlines through the Gateway Student Success Center. You can find the form on-line at: http://nau.edu/VPAA/University-Advising/Reinstatement/.
