Was there a pre notification of contact details of PFSO/ Points of contact in case of breach of security ?

SECURITY EXCHANGE – PRIOR COMMENCEMENT OF OPERATIONS Was PFSO or his representative present to sign ship's DOS? Was ship's DOS accepted or terminal insisted on using only its own DOS? Was a dedicated means of security communication used? Interaction with Authorities Was vessel subject to pre-arrival boarding in stream or prior berthing? Was adequate identification provided by the officials boarding the vessel ? Was there any crew mustering/interrogation carried out? Were there any problems encountered with specific crew nationalities being targeted? Were there any perceived Ship Security Deficiency issues ? Was there any additional information demands from the port state inspector ( e.g. security plan disclosure issues?) SHORE LEAVE ACCESS FOR CREW Was ship's crew allowed to go ashore while alongside? Was ship's crew allowed to use terminal facilities to go ashore? Were there telephones on berth for crew use? Was there any kind of charges involved to go ashore? Was there a requirement of any security services to be hired for crew to go ashore? was an alternate means of transportation readily available if the terminal did not allow taxi's to come inside their facility? Was shore access permitted at no charge in case of a medical emergency?

SHORE ACCESS FOR VISITORS / CREW REPATRIATION Were company representatives, vetting inspectors, surveyors, technicians, repair men etc allowed to come on board? Was crew repatriation permitted using terminal facilities? Was crew baggage allowed to pass through the terminal? Was all baggage of crew / visitors screened on entering terminal? Does terminal provide vessel with a visitors list for terminal reps or Authorities boarding the vessel? Are friends/relatives of crew allowed to board vessel with prior notification?

ACCESS OF SHIP’S STORES Was vessel allowed to receive stores / provisions whilst alongside? Were all stores / provisions screened ashore prior delivery ? Was any kind of charges involved to receive stores/provisions?

SHORE ACCESS FOR OPERATIONAL REASONS . Was shore access permitted to carry out operational checks eg taking drafts ? ADDITIONAL ITEMS . Any particular incident/situation encountered by vessel which impacts the security of the vessel? Any objections raised by the third parties about the security measures being implemented on board?
