It is my pleasure to offer you a position as a fellow in ______at the University of Utah Health Sciences Center beginning July 1, 2___. Everyone in the Division enjoyed meeting you and felt that you would be an excellent addition to our Fellowship Program. The Fellowship salary schedule beginning in July of 2___ states that your stipend amount will be ______(PGY_). While we have every confidence in your future success, we are required to inform you that if you engage in conduct prior to your start date and such conduct, were you employed by the University of Utah, would subject you to disciplinary action under University of Utah policies, including but not limited to Graduate Medical Education policies, this offer may be rescinded.

As part of the credentialing process for our new hire housestaff, you will be asked to authorize a criminal background check on yourself and to undergo drug testing. Drug testing will occur at your New Hire Orientation here at University of Utah. A criminal background check will be performed prior to your hire date. Please be aware that you will be contacted via email by our secure, authorized background check provider, Accurate Background Checks. On a very secure site, you will be asked to submit specific information and to authorize the background check. Please cooperate with this process. You cannot be hired until the Graduate Medical Education Office has received clearance from our Human Resources department. In an electronic “Welcome Packet” that will be emailed to you in early April, you will receive more information regarding the criminal background check. I am enclosing a copy of the current contract for your information only. The new contracts will be available in January and we will have one ready for your signature at that time. In addition, our fellows are eligible for a number of benefits. I have also attached a copy of the current resident stipend memos that provides a brief description of these benefits.

In addition, the Division will provide a _____ stipend to use for books, journals and travel for presentations at professional meetings. We look forward to hearing from you and hope that your move to Salt Lake City is uneventful. We welcome you to our program.
