Dear Registered or Licensed Child Care Provider,

I am sending this letter and our new Referral Service Option Provider Agreement, Part 2 (questionnaire) on behalf of your local Child Care Referral Specialist. Referral Specialists help families find child care options that exist in their communities. This Agreement applies to registered and licensed child care providers only. It does not apply to legally exempt child care providers.

Referrals to families includes profiles of providers from the information that is supplied by you. By completing the optional information on the enclosed Referral Service Option Provider Agreement, Part 2 and returning it to your local child care referral specialist as soon as possible, you can offer additional information to the public and to child care referral specialists working with the public to better inform them about your child care setting. You can use this expanded referral information to promote your business. Please note that the expanded referral information feature in BFIS will not be activated until July 1, 2005.

Even if you aren’t at the point where you want to be receiving referrals, please fill it out and return indicating that you currently don’t want to receive referrals. Our new Bright Futures Information System (BFIS) allows your Referral Specialist to mark your personalized provider profile as inactive, which can always be changed in the future should you want to switch it.

If you have questions about the Agreement, please call your Child Care Referral Specialist. She would be happy to chat with you. If you would be interested in a copy of your profile, she can send one at your request.

Please return your Referral Provider Agreement, Part 2 to your Child Care Referral Specialist as soon as possible. See the enclosed Community Child Care Support Services Agency list for your Child Care Referral Specialist’s mailing address and phone number.
