PROGRAM FOR CHILDREN AGES 13 MONTHS TO 19 MONTHS

The Ladybugs Program is a YEAR-LONG program beginning Thursday, September 7, 2017 and ending August 17, 2018.

YOU MAY CHOOSE ONE (1) to FOUR (4) DAYS OF ENROLLMENT; INFANT CARE IS AVAILABLE FROM 7:30 A.M. TO 6:30 P.M.

Tuition is based on which days of the week your child attends.

IF YOUR CHILD ATTENDS ON A TUESDAY, WEDNESDAY OR THURSDAY, THE ANNUAL TUITION IS $5,210 PER DAY.

IF YOUR CHILD ATTENDS ON A MONDAY OR A FRIDAY, THE ANNUAL TUITION IS $5,090 PER DAY.

Upon your child’s acceptance into the MMO LADYBUGS PROGRAM, a contract will be issued. Your child’s space will ONLY be secured upon submission of the contract and NON-REFUNDABLE DEPOSIT OF five hundred dollars ($500.)

In order for your application to be considered, you must:  Return this form filled out in its entirety  Submit an $85 NON-REFUNDABLE processing fee.

MMO Programs reserves the right to place children in appropriate class groupings, with regard to age and gender balance.

Students are required to be fully immunized to attend MMO, unless a medical contraindication exists. As a center affiliated with a religious institution, MMO is not required to allow religious or philosophical exemptions.

Deadline for priority registration (Church members, returning children and their siblings) is January 20, 2017. Priority and open enrollment applications will be considered in the order they are received. Waitlisted applications will remain valid through August 17, 2018.
