AFROTC FORM 48: YOUR PLAN TO GRADUATE ON TIME

PURPOSE:

The academic plan is designed to ensure each applicant meets graduation and program completion requirements in a realistic manner. Once the academic plan is established, at a minimum all cadets must have their academic plan re-evaluated and re-certified by their academic advisor every fall term.

You have until the end of your first academic term in the AFROTC program to turn in a completed FORM 48. Failure to turn in an academic plan may result in dismissal from the AFROTC program or placement in special student status.

Changes to the original FORM 48 may only be made for course changes. If you change the year you are graduating or change majors, then you must complete a new academic plan FORM 48.

INSTRUCTIONS:

Follow these instructions step-by-step to complete your FORM 48. Your form is kept as part of your permanent record. An appropriate institutional official must review, verify and sign BLOCK 4. Return the document to detachment cadre after completion.

SECTION I:

#1 – NAME: Last Name, First Name and Middle Initial

#2 – ACADEMIC INSITITUION & AFROTC DETACHMENT: Enter the name of the university where you will receive your degree / Det 280

#3 – ACADEMIC MAJOR: Enter major field of study. If you change your major, you MUST complete a new FORM 48. NOTE: If you are on scholarship/contract or have been submitted for a scholarship or enrollment allocation to enter the POC, you must obtain approval through the Detachment Commander PRIOR to changing your academic major.

#4 – INSTITUIONAL OFFICAL REVIEW: Your FORM 48 must be signed and date by a dean, department chair, faculty advisor or other appropriate institutional official. His/her signature/date verifies that the plan, if successfully completed, will lead to the prescribed degree at the time indicated.

A new institutional official review is required if a new FORM 48 is completed. #5 – INITIAL REVIEW: Complete the statement indicating the specific degree (BA, BS) and the month and year your degree will be awarded.

#6 – DEGREE REQUIREMENTS: This block is signed and dated the week prior to your commissioning.

SECTION II:

List all courses you plan to take each term by course number, title and credit hours. Start with any transfer credits or courses from previous semesters. Then list your first semester in the AFROTC program and list all terms required to complete degree and commissioning requirements from left to right on the form. List summer semester each academic year, you will leave the course listed blank if not taking any courses that summer, however you must list the summer session. List Field Training.

COURSE NUMBER: Enter the course number as listed in the school catalog.

COURSE TITLE: Provide the title of the course

CREDIT HOURS ATTEMPTED: Enter # of hours specified in the course catalog

CREDIT HOURS COMPLETED: This block will be completed by your AFROTC instructor

DEVIATIONS: This block will be completed by your AFORTC instructor

TUITION VERIFIED: This block will be completed by your detachment cadre

CREDIT HOURS ATTEMPTED: Enter # of hours specified in the course catalog

TOTAL CREDIT HOURS ATTEMPTED/COMPLETED: This block will be completed by your AFROTC instructor

FALL TERM EVALUATION COMPLETE: All cadets must have their academic plan re-evaluated and re-certified by their academic advisor every fall term.

ADDITIONAL NOTES:

- All Aerospace Studies course (i.e. AIR and LLAB) will be listed in the term they will be taken.

- You may list any elective course as “elective” if you do not know the course title at the time the form is being completed. You will add the actual course title when it becomes available.
