We would like to invite you to come into the practice for an NHS Health Check.
The Department of Health recommends looking at possible risk factors for heart disease and stroke in patients who are 40 years or older. Many heart attacks or strokes could be prevented with simple measures such as eating more healthily or stopping smoking.
We would like to support you in being as healthy as possible and would like to invite you to come to the practice for an NHS Health Check. This involves looking at your blood pressure, your cholesterol and your weight for example, and you have the opportunity to talk to the nurse about how you could improve your heart health and discuss any concerns you may have.
The appointment will last approximately 15 - 20 minutes.
What you need to do next:
1. Please call the practice on 01275 841630 after 10.30 a.m., saying that you have received this invitation and ask to book in for an NHS Health Check. Please give code “9MC”. 2. Complete the enclosed double-sided form. Please arrive 10 minutes before the start of your appointment and use the blood pressure machine in the waiting room to obtain the ‘SYS’, ‘DIA’ and ‘PUL’ readings. 3. Please bring the fully completed form to your appointment as the nurse will need the information.
We hope to see you at the practice. Alternatively call into any of the pharmacies listed overleaf, with this letter, and they will be able to carry out the NHS Health Check for you.
