PRE-REQUISITES IN RESPECT OF APPLICATION TO BE APPROVED AS SUITABLE PERSON OR BODY OF PERSONS TO BE ABLE TO APPLY FOR REGISTRATION OF TESTING STATION {Section 38 of the National Road Traffic Act, 1996 (Act no. 93 of 1996) read with Regulation 128 of the said Act}

 Affidavit / affirmation declaring such intentions to operate a Testing Station which shall contain the following:

 particulars of every person, or the members or directors of every juristic person in whose name such Testing Station shall be registered in the event that such application is approved,  the exact location of the proposed Testing Station, and  the grade of testing station to be operated.

 Proof of payment of the applicable fee to determine the suitability of the person or body of persons,

 Written resolution from the Municipal Council of the municipality in which the Testing Station is located, supporting the application of the Testing Station,

 Motivation for the need for the Testing Station / feasibility study conducted by the person or body of persons,

 copy of the initial and amended founding statement, partnership agreement, close corporation registration or company registration reflecting the sole proprietor, all partners, all members or all directors, respectively, of the Testing Station,

 certified copy of the South African identity documents of the sole proprietor, all partners, all members or all directors, as is relevant,

 a current South African Police Services clearance or SAPS 69, reflecting any criminal record and the details and nature of any offence or offences, for the sole proprietor, all partners, all members or all directors, as is relevant, and

 a current tax clearance certificate in the name of the person or body of persons making application, reflecting income tax registration number and value added tax number.

REQUIREMENTS IN RESPECT OF MANNER OF APPLICATION OF APPROVED PERSON OR BODY OF PERSONS TO REGISTER TESTING STATION {Section 38 of the National Road Traffic Act, 1996 (Act No. 93 of 1996) read with Regulation 130 and 131 of the said Act}

 Form TS1,

 Affidavit from Testing Station confirming the written appointment and relevant details of its Management Representative and alternate Management Representative, and where the Management Representative, alternate Management Representative is not the owner of the Testing Station, that the Management Representative, alternate Management Representative is acting on behalf of the Testing Station,

 certified copy of the South African identity documents of the Testing Station Management Representative, alternate Management Representative and all examiners of vehicles,

 certified copy of all examiners of vehicles registration certificates,

 a current South African Police Services clearance or SAPS 69, reflecting any criminal record and the details and nature of any offence or offences, for the Management Representative, alternate Management Representative and all examiners of vehicles,  affidavit from Testing Station confirming the residential address of the sole proprietor, all partners, all members or all directors, Management Representative, alternate Management Representative and all examiners of vehicles,

 copy of the title deed, offer to purchase or lease agreement of the premises on which the Testing Station is located,

 affidavit identifying any:

 prior entity providing the same or similar services in which the Testing Station or its owners, members, directors, Management Representative or senior management have had an interest in the 10 (ten) year period prior to this application; and  prior activities of a same or similar nature to the Testing Station in which the Testing Station owners, member, directors, Management Representative or senior management have been engaged in the 10 (ten) year period prior to this application,

 Letter identifying the signatory of the approved person or body of persons for the purposes of entering into the Agreement (mandatory for all Test Stations to enter into an Agreement with the Department),

 Proof of payment of the applicable fee for the registration of the Testing Station,

 Inspectorate of Testing Stations (SABS) recommendation (once the Test Station is established the Department will request the Inspectorate of Testing Stations to evaluate the Test Station).
