Cost of Trip: $ 225 Checks should be made payable to Shawnee Band Parent’s Association and are due By Friday, January 8th
Includes: 2 nights at the Hilton Back Bay Boston all ground transportation festival fees
Not Included: NO MEALS INCLUDED IN PRICE! I would figure $25 a day for meals Medicine/ Illness All medical paperwork needs to be completed by December 22nd. You do not need to resubmit Doctor’s orders if you have them on file with the Nurse from Band Camp, however, everyone must resubmit the Health History and Release of Claims. Please note, a Doctor must sign off on all medication, including Tylenol, advil, birth control, allergy medicine, and antacids. Students are not allowed to carry ANY medication. You will give the medication to an administrator before we depart Shawnee. A nurse will administer all medication. Any student caught with any medication, except for inhalers or epi pens, will be immediately sent home and reported to Administration for disciplinary action.
Student will be taken to the nearest hospital in the event of an illness or emergency.
Meals: Please note that meals are not included in the cost of the trip.
Spending Money: Will vary by appetite and spending habits of the individual. Students will have time to shop in Faniel Hall and the mall attached to our hotel.

BERKLEE PERMISSION SLIP 1. All regulations for student behavior in school which can logically be applied to the music trip will be in effect. This also stands as a reminder to any students who are 18 years of age or older that they are still under the rules and regulations of the school.
2. The school has designated chaperones to take full charge of the students and of all details of the trip from the time of departure until the time of return to S.H.S. It is understood that the chaperones will have full control of the movements and control of each individual student. In case of difficulty with any student, appropriate disciplinary action will be taken. Disciplinary action may include restriction from participating in scheduled activities. If the difficulty is deemed to be extremely serious, the student will be sent home, accompanied by a chaperone if possible. If it is not possible to send a chaperone with the student, it will then become the responsibility of the parent or guardian to make arrangements to transport their student back home (come and pick them up!). The parent or guardian will also be financially responsible for this transportation. (both the student & chaperone.) Students should understand that the chaperones are present to help them.
3. Students are to report any accidents or illness immediately to a chaperone.
4. Students are to report to the high school at the designated time and date. All luggage must be at the High School the morning of departure and should be tagged and UNLOCKED!, Luggage will be administration before boarding the buses. Each person is allowed one suitcase and one carry on.
5. Students do not need an extensive wardrobe for the trip. The key words are "comfort" and "casual" although there is a difference between being dressed casually and sloppily. During the day you will be doing a lot of walking, so comfortable walking shoes are a must
6. Students who take medication must have submitted the proper paperwork, and must have the medication in a prescription bottle. 7. Possession, purchase and/or use of Alcoholic beverages of any kind, and/or the possession, purchase and/or use of drugs other than those for medical purposes previously reported to the staff, is not permitted at any time. Anyone who consumes any of these items while participating in a school activity, on or off the school grounds, shall be subject to disciplinary action, & the jurisdiction of the authorities.
8. To avoid any problems, all forms, & financial obligations must be turned in immediately.
9. Every effort is being made to provide ultimate security and safety for all participants on this trip and for all the equipment and personal belongings going along with it. Every participant agrees that in case of an accident resulting in personal injury to any student during the trip, the school and chaperones will not be held responsible. In the event of illness or family emergency which would require that a student be sent home before the termination of the trip, the student will be sent home by the best available means of transportation, accompanied by a chaperone if possible. If that is not possible, it will be the responsibility of the parent / guardian for transportation home. In the event of hospitalization or other problems, It will be the parent / guardians responsibility to go to the hospital and make arrangements home. Financial responsibility will rest with the parent / guardian.
10. Students will be given an Itinerary listing all the events and times, and they will be required to follow the Itinerary and be on time for all events listed.
11. Before leaving the buses each night students must check to see that no personal belongings are left on the bus.
12. Students that are sharing a room are responsible for anything that happens in that room. The cost of damaged or missing Items will be shared equally by those who occupy the room unless the responsible person(s) assume the responsibility on their own. Prior to leaving the hotel, each room will be checked for any missing items and / or damages.
13. Each student must be in his / her room at the end of the days events at the time given by the chaperones. No one should leave their room after that time until wake up call.

15. Students had the chance to choose rooms. Changes won’t happen barring emergency!
16. In maintaining the good name of SHS, the group will conduct themselves in a credible manner at all times.
17. No large radios / boxes will be permitted on the trip at any time. Walkmen / Discmen with headphones may be used on the Motor coaches, or in the rooms.
18. Students are not permitted on other floors of the hotel except the ones that we have been assigned.
