PUBLIC MEETING MINUTES: EXAMINING BOARD OF PHYSICAL THERAPISTS AND ATHLETICS TRAINERS

Mr. Nowell called the meeting to order at 5:09 p.m.

REVIEW OF MINUTES

The board reviewed the minutes of the February 21, 2006. Mr. Kotrch made a motion to approve the minutes as amended, seconded by Ms. Messick. The motion was unanimously carried.

UNFINISHED BUSINESS

Review of Request for Special Accommodations The board reviewed the request for special testing accommodations from Colleen Butcher. The board determined that after reviewing the additional information from the physician, which included the DSM-IV code, that Ms. Butcher is approved to take the NPTE with special accommodations.

Hearing – Complaint #14-09-04 – Emund Oliveros, PT Mr. Nowell opened the hearing at 5:32 p.m. Ms. Adams began the hearing by reading the complaint against Mr. Oliveros. Mr. Hanley presented the State’s case, which included one exhibit. Mr. Oliveros presented his own defense, presented one exhibit and answered questions from the Board. The Board recessed from the hearing at 6:26 p.m. for deliberations. The hearing reconvened at 7:21 p.m.

Mr. McKenzie made a motion, seconded by Mr. Kotrch, to revoke the licensure of Mr. Oliveros. The motion was carried by a vote of 5 – 2, with Ms. Whiteman and Ms. Smith opposing.

Ms. Adams will supply the order to be signed at the next board meeting. Mr. Nowell closed the hearing at 7:25 p.m.

NEW BUSINESS

Review of PT/PTA/AT Applications by Reciprocity Ms. Whiteman made a motion, seconded by Mr. Kotrch, to approve licensure for the following persons:

Lisa H. Goodfriend John G. Jurjans, Jr. Susan U. Kush Lise P. Meloy Deanna L. Pasch Kelly L. Rostad Peter W. Teaney Lucas L. Wachewicz Examining Board of Physical Therapists and Athletic Trainers Minutes –March 21, 2006 Page 3 The motion was unanimously carried.

Review of PT/PTA/AT Applications by Examination Ms. Whiteman made a motion, seconded by Mr. Kotrch, to approve licensure for the following persons: Tamara R. Blusk Aaron J. Cooke Cara Marzano Megan E. Moeller Travis M. Ross Michael A. Voltz Henry G. Wright

The motion was unanimously carried.

The application of John G. Jurjans, Jr. for Athletic Trainer by examination was tabled.

Review of Applicant Credentials Ms. Whiteman made a motion, seconded by Mr. Kotrch, to propose to deny licensure due to lack of education credentials for the following person:

Tracy M. Cristomo

The motion was unanimously passed.

Review of Test Results Ms. Whiteman made a motion, seconded by Mr. Kotrch, to recognize examination results for the following persons:

Ma Czarina M. Dulay Christy L. Sullivan Albert D. Viray

The motion was unanimously carried.

Review of Continuing Education Activities Mr. McKenzie made a motion, seconded by Ms. Whiteman, to approve or deny the following courses as noted:  National Seating and Mobility – The Challenge of Change – 1.40 CEUs  Columbia University College of Physicians & Surgeons – Center for Continuing Education (Patricia O’Shea) – Neuromuscular Issues in Children – 0.60 CEUs  Physical Therapy Dept. at St. Barnabas Medical Center (Patricia O’Shea) – Intensive Care Unit Update & Critical Care Polyneuropathy – 0.675 CEUs  Pro Physical Therapy (Paul Vidal) – Evidence Based Considerations for Practice: The Shoulder Complex – 0.75 CEUs  Office of Health Facilities Licensing and Certification & Quality Insights of Delaware – OASIS: A Reflection of Patient Care – 0.45 CEUs Examining Board of Physical Therapists and Athletic Trainers Minutes –March 21, 2006 Page 4

The motion was unanimously carried.

Adjournment

There being no further business, Mr. Kotrch made a motion, seconded by Mr. McKenzie to adjourn the meeting. The motion was unanimously carried. The meeting adjourned at 7:35 p.m.
