1. Tan Tock Seng Hospital invites your quotation for the Appointment of an Operator to install and operate a self-service automated machine at Tan Tock Seng Hospital (TTSH).

2. This document consists of the followings (total 7 pages):

a) Invitation to quote (this page) (1 page); b) Annex A: Specifications (4 pages); c) Annex B: Vendor Address & Contact Person (1 page); d) SAM Proposed Location (1 page)

3. Quotation shall be in accordance with the hospital’s Standard Conditions of Contract SCC.3 A copy of the hospital’s Terms and Conditions can be downloaded from the Quotation’s Web Site via http://www.ttsh.com.sg/asp/ten/ten42.asp

4. Please sign the last page of the Conditions of Contract SCC.3 and send in together with your quote submission to us.

5. It shall assume that the quotation meets the specifications in all aspects notwithstanding any reference to accompanying descriptive brochures / pamphlets unless The Contractor clearly states otherwise and indicate the areas in which the quotation do not meet specifications in Annex B. All requested information must be duly provided. Failure to do so might result in disqualification of the quotation.

6. Please submit 2 copies of quotation proposal together with catalogs.
