8. All members must commit to two roles during the season. Some performers act in 3 shows in the season. Some gluttons for punishment perform in all shows. It is really a matter of what works best for you. Being realistic, how many shows can you be in, in Season 2005?

By signing and submitting this application, I certify that the above information is true and correct to the best of my knowledge. I have read and understand The Document of Understanding for MTI season 2009. I further understand that if I am chosen to participate in the 2009 company, that there are membership fees applicable. I understand that it is not the intention of the MTI to exclude participation based on economic conditions, therefore, if I cannot pay the fee, I will inquire with regards to the MTI fee reduction program. No payment is due at this time. This form is only an application, once you are accepted into the company, you will be advised of fee payment arrangements. Further, by completing and signing this application, the participant and their guardian acknowledge full understanding that participation in MTI activities is on a voluntary basis. Participants are not paid for their services. Participants ARE NOT employees of the company and are not covered for liability under workers compensation insurance. All participants understand that the MTI seeks to provide the safest possible environment, however, you participate “at their own risk.” Participants agree to hold blameless the Music Theatre of Idaho, The Nampa Civic Center and/or The City of Nampa or any employee of the noted businesses or entities for any accident or injury incurred while participating in MTI activities. Once cast in the MTI company, each participant will be required to provide the MTI with proof of individual medical insurance. If a participant does not carry insurance, then a further release of liability will need to be signed by the participant or their guardian.
