2011 - 2012 ACG Texas Benefits Overview For Salaried Employees

ACG Texas offers a comprehensive benefits package to meet your health and wellness needs. As a full-time salaried manager, you will be eligible for benefits on the first day of the month after 90 days of employment.

During your first weeks of employment, you will receive an email from Benetrac with log-in information to complete your benefits enrollment. Benefit payments are made through a bi-weekly payroll deduction. Medical, dental, and vision care benefits are withheld on a pre-tax basis; supplemental life insurance and short-term disability are deducted on a post-tax basis.

The information below will explain all of the benefits ACG Texas offers to help you make the selections that best meet your needs and your budget. Medical Insurance Blue Cross Blue Shield of Texas provides 3 levels of medical insurance for individuals and families. Each medical plan is described below.

Plan A - M11 - Medical Insurance  No cost for routine exams; $30 co-pay for specialists.  Unlimited lifetime benefit for medical care.  Deductible is $1,000 for individual or $3,000 for family - in network or out-of- network.  Co-insurance pays 80% for in-network or 60% for out-of-network.  Maximum annual out-of-pocket cost is $5,000 for individual or $15,000 for family with an in-network provider. The chart below shows the bi-weekly cost for Plan A Medical Insurance. As ACG Texas pays the greatest share of the cost for medical insurance, the amount you will pay is found in the column labeled 'Your Cost.'

Plan B - M26 - Medical Insurance  No cost for routine exams; $40 co-pay for specialists.  Unlimited lifetime benefit for medical care.  In network or out-of-network deductible is $3,000 for individual or $9,000 for family.  Co-insurance pays 70% for in-network or 50% for out-of-network.  Maximum annual out-of-pocket cost is $8,000 for individual or $24,000 for family with an in-network provider. The chart below shows the bi-weekly cost for Plan B Medical Insurance. As ACG Texas pays the greatest share of the cost for medical insurance, the amount you will pay is found in the column labeled 'Your Cost.'

Plan C - MH2 - HSA (Health Savings Account) Medical Insurance  No cost for routine exams.  Physician and specialists' visits are subject to deductible.  Unlimited lifetime benefit for medical care.  Co-insurance pays 100% for in-network or 70% for out-of-network.  In network deductible is $3,000 for individual or $6,000 for family. Dental Insurance MetLife provides 2 levels of dental insurance for individuals and families. Each plan is described below.

MetLife Dental Choice Insurance MetLife Dental Choice is a traditional PPO dental program.

The chart below illustrates that a routine visit to the dentist, a periodonic exam, and ex- rays will cost nothing. For extractions, fillings, and root canals, MetLife will pay for 80% of the service and the patient will pay 20%. For crowns and surgical implants, MetLife will pay for 50% of the service and the patient will pay 50%.

The orthodontic benefit per covered child is $1,500.

MetLife DHMO Dental Care Insurance MetLife DHMO Dental Care takes the guess-work out of dental cost by providing an up- front out-of-pocket schedule for every service provided.

The chart below illustrates the out-of-pocket cost for services. For example, a routine dental visit will cost $5 while a periodonic exam and ex-rays will cost nothing. An extraction will cost $5, a filling will cost $12, a crown will cost $290, and a root canal will cost $330.

Participants in the DHMO plan must log into MetLife at www.metlife.com/dental to select a primary dentist. Enter plan name SGX290 and your zip code to find a dentist near you. Each dentist will be identified with a 5-digit facility number. Next, contact Metlife at 1-800-638-5433 and provide the dentist's 5-digit facility number to establish your primary dentist.

Vision Insurance MetLife EyeMed vision insurance is widely accepted by professional eye care facilities nationwide including LensCrafters, Pearle Vision, Sears Optical, Target Optical, and JCPenney Optical. Discounts are available for Lasik treatment through LCA Vision.

Healthcare Advisor Compass provides a staff of Professional Health Advisors to help participants with healthcare needs. They will help determine which physician or hospital will provide medical services at the lowest out-of-pocket expense. They will also recommend various treatment options, obtain medical records, and reconcile physician's invoices and statements.

The Compass staff is available 24 hours a day, 7 days a week. You can reach them by phone at 1-800-513-1667 or by email at [email protected]. Life Insurance A $25,000 life insurance policy is provided at no cost to full-time salaried managers of ACG Texas through Lincoln Financial.

Guaranteed acceptance of voluntary life insurance up to $100,000 is available to first- time enrollments.

Supplemental life insurance may be elected in increments of $10,000 up to $200,000 for participants and is also available for spouses or dependent children.

The plan includes portability, conversion, waiver of premium, an accelerated death benefit, and a travel accident policy. Premium cost is calculated on individual demographics.

Lincoln Financial also provides an Employee Assistance Program (EAP). Long Term Disability Long-term disability insurance is provided at no cost to full-time salaried managers of ACG Texas through Lincoln Financial. Coverage provides for 60% of a participant's monthly salary up to $10,000 per month. Short Term Disability Short-term disability insurance is an elected benefit available to full-time managers through Lincoln Financial. Coverage provides for 60% of the participant's weekly earnings with a maximum pay-out of $1,000 per week. Premium cost is calculated on individual demographics.
